During his teen years, Hill began to isolate himself from "outside-of-class pursuits." He spent considerable time online in the hopes of building a following as an internet celebrity to promote his work, but was uncertain what to concentrate on creatively. "I was doing Facebook comedy videos, then I went on Instagram, and then I hopped on Twitter, where I really was a master. "It's the first place where I could go viral." He also made short-format comedy videos on Facebook and Vine.

According to a New York Magazine probe, he reportedly created and maintained Nicki Minaj fan pages on Twitter, including one called "@NasMaraj." This website attracted attention on Twitter in 2017 for its flash fiction-style interactive "scenario threads" popularized on Twitter, thanks in part of the dashboard software TweetDeck. The probe linked @NasMaraj to the usage of "tweetdecking," or using multiple accounts in collaboration to artificially make certain tweets go viral. Due to "violating spam policies," @NasMaraj's account was banned on Twitter. After the suspension of @NasMaraj, New York Magazine's probe revealed that he opened a new account with the name "@NasMarai" and that his current Twitter account at the time was a repurposed version of the "@NasMarai" account with a changed handle. Lil Nas X was linked to the Minaj fan pages by media outlets, but he dismissed them as a "misunderstanding," effectively denying that he had been running the accounts. However, Lil Nas X confessed to being a Minaj fan in May 2020 in a tweet. He explained why he initially denied it, saying that if people knew he was a fan of hers, they would assume he was gay: "People will guess if you had a whole fan page dedicated to nicki u are gay." And the rap/rocket industry hasn't exactly created or accepted gay men yet." Minaj responded to Nas on June 17, 2020, saying, "It was a sting when you denied being a barbarian." I understand. Congratulations on the courage you have gained to speak your truth. Lil Nas apologised to Minaj, saying he "felt so bad, wishing u wouldn't see my denial." The @nasMarai account was later revealed in a New York Times Magazine article, which described Hill as having spent "every waking hour online, tweeting as @nasmaraj." It is also included in Lil Nas X's music video, which shows him growing up as a closeted teen and celebrating his sexuality; in high school, he is seen tweeting from "nasmiraj" as the song "I'd be by the phone, stanning Nicki morning into dawn" plays.

Hill began writing and recording songs in his closet during the year 2018. He gave the name Lil Nas X, which is a nodoe to rapper Nas. Lil Nasarati's first mixtape Nasarati appeared on SoundCloud on July 24, 2018, but it was not immediately known; Nasarati would be taken from streaming platforms shortly after the debut of Old Town Road due to a copyright war, and would therefore never gain much traction. He was surprised to hear that "Old Town Road" would become "Old Town Road" in late October 2018.

Lil Nas X's "Old Town Road" became a country rap song on December 3, 2018. He bought the beat for the song from Dutch producer YoungKio for $30; it features Nine Inch Nails' track "34 Ghosts IV" from their sixth studio album Ghosts I–IV (2008). He appeared on "humble" Atlanta's CinCoYo's "$20 Tuesdays" on Tuesdays, with less than an hour. Lil Nas X began making memes to advertise "Old Town Road" before it was picked up by TikTok users in a short-form video social network. TikTok has urged its 500 million global viewers to "endless imitate" by making videos; the "app's frantic churn of content... acts as a potent incubator for viral music hits." Lil Nas X estimated that he made about 100 memes to promote it; the song went viral in early 2019 as a result of the #Yee Haw Challenge meme on TikTok. Millions of people shared videos of themselves dressed as a wrestler or cowgirl, with the majority of #yeehaw clips using the song as their soundtrack; as of July 2019, they have been watched more than 67 million times. Children are also included in the statistics of adult listeners, and they are a key audience tied to social media. According to Quartz.com, the song certainly owes a part of its popularity to the age, and children are attracted to the song being repetitive, easy to sing, and using lyrics about riding horses and tractors, which children can relate to. On the Billboard Hot 100 chart, it debuted at number 83, before gaining to number one. The album debuted on the Hot Country Songs chart at number 19 and Hot R&B/Hip-Hop Songs at number 36. Lil Nas X joined Columbia Records in March 2019 after a "intense bidding war." Billboard and Rolling Stone reported that Billboard mysteriously dropped the song from the Hot Country Songs chart in March 2019.

"Taking 'Old Town Road' off the country chart shows me as racist, simple and straightforward, because the country radio network is still white, regardless of the Darius Ruckers and Kane Browns it accommodates." "Billboard's decision to pull the song off the country chart had absolutely nothing to do with the artist's ethnicity," a Billboard spokesperson told Genius. Despite being taken off the main Country Songs chart, the song debuted on Billboard's Country Airplay chart, debuting at 53 years old and peaking at 50. Sony Music Nashville CEO Randy Goodman told Billboard that his staff started testing the music in some country radio markets, adding that "it would be negligent not to look at it." When Wrangler introduced its Lil Nas X collection in May 2019, the discussion of racism in country music culture resurfaced, and some customers threatened to boycott. The song, according to media outlets, also draws attention to the enduring cultural era of African-Americans from both country music and the American frontier period.

Billy Ray Cyrus, a country music legend, starred on "Old Town Road" and became the first vocalist in a April 2019 remix, the first of many. Lil Nas X set a record for the most U.S. streams from one week in one week in July 2018, beating Drake's "In My Feelings," which had 116.2 million streams in a week, as of August 2019; it has more than 1 billion plays on Spotify alone. The video was released in May 2019 and has over 370 million views as of August 2019. "In the social media age, Lil Nas X is arguably the first micro-platform crossover celebrity," NBC News' Michael Arceneaux wrote.

On June 20, 2019, Lil Nas X's second single, "Panini," was released as Lil Nas X's second single from Columbia Records. It is named after the fictional cabbit of the same name in the animated television series Chowder, but does not refer to a sandwich of the same name. "Panini" was the first remix to be released in mid-September 2019 with rapper DaBaby. Lil Nas X's debut extended play, titled 7, was published on June 21, 2019. The EP debuted on the Billboard 200 charts at number two. Lil Nas X was a judge at the 2019 BET Awards on June 23, 2019. Lil Nas X made his international debut on June 30th at the United Kingdom's annual Glastonbury Festival, where he and Billy Ray Cyrus surprised audiences and joined Miley Cyrus for the show before performing "Panini" solo in a set seen nationally on BBC. When Lil Nas X came out as gay, he became one of the most prominent Black queer male singers on the day. This was particularly relevant for an artist in the country and hip hop styles, both of whom emphasize machismo and "historically snubbed queer artists." Frank Ocean's black queer male artists in hip hop were gaining mainstream recognition in 2012, well before Channel Orange was announced. Three Rolling Stone Top 100 hit the charts in early July with three Lil Nas X songs: "Rodeo" with Cardi B at number nine; "Panini" at four; and "Old Town Road" as the first-ever number one track.

Lil Nas X's debut album, "almost finished," he said on July 7, 2020. He also said he was working on a mixtape and encouraged producers to contribute their beats for his new music. He teased the debut of "Call Me by Your Name" on July 10, tweeting a snippet of it online. "Holiday," his new single, which was released on November 8, 2020, was announced on November 8. C Is for Country, a children's book, was published in January 2021. In a Super Bowl LV commercial the following month, he reviewed the song "Montero (Call Me by Your Name)" once more. The album and a accompanying music video were announced on March 26, 2021. Lil Nas X announced on the same day that his debut album Montero would be announced and that it would be announced in mid-2021. The video sparked a lot of emotion. Later, he released a prelude video for the album "Industry Baby," which staged a fake "Nike vs. Lil Nas X" court hearing in which people debate the Satan Shoes before chastising him for being gay.

Lil Nas X's latest single "Sun Goes Down" delves into his teenage years and its reception, after the scandal surrounding his previous work and its promotion, wherein he addresses his struggles with bullying and coming to terms with his homosexuality in his upbringing. He appeared on Saturday Night Live alongside "Montero (Call Me By Your Name)" a day later, where he suffered a wardrobe malfunction during a pole dance routine, leaving him unable to finish it properly. Lil Nas released a promotional video of his debut album on June 29, delving with Montero, the album. In addition, he teased a snippet of a previously teased track named "Industry Baby."

Lil Nas X released a video on TikTok a day earlier announcing that he had a court hearing on the Satan Shoes three days later. On July 19, 2021, he posted a spoof of the legal mess on YouTube, promoting his new song, "Industry Baby." The album was released on July 23, starring rapper Jack Harlow, and Kanye West and Take a Daytrip. The song debuted at number two on the Billboard Hot 100 and debuted at number one on the chart for the fourth week of October 23, 2021, Lil Nas X's fourth top ten single and third number one.

Montero was announced on September 17, 2021, along with its fourth song, "Thats What I Want." Lil Nas X made a surprise appearance at Diplo's set at Electronic Daisy Carnival Las Vegas on October 23, 2021. During the appearance, he performed "Industry Baby," "Montero (Call Me by Your Name)," and "Old Town Road."

He announced the Long Live Montero Tour on April 26, 2022, his first concert tour. The tour will debut in September 2022 and run into November 2022 in favor of Montero.

Lil Nas X returned from his hiatus on March 16, 2022, and the first two songs from his "almost finished" debut, "Late to da Party" featuring YoungBoy Never Broke Again, and "Down Souf Hoes" featuring Saucy Santana appeared on "Like Again." He also gave a preview of the track "Lean on My Body." Lil Nas X launched "Late to Da Party" on June 24, 2022, but later revealed on Twitter that it was "not a lead single." According to Lil Nas X, the album would be "something exciting, something for the summertime, and something for the girls to look forward and enjoy."

Riot Games, the makers of the video game League of Legends, announced a partnership with Lil Nas X for the 2022 League of Legends World Championship on September 15, 2022, where they also announced him as "President of League of Legends" in a press release. On September 22, the tournament's anthem, "Star Walkin," was announced, as well as an accompanying animated music video. On November 5, he will perform the song at the tournament's opening ceremony.
