Williams also designed for a "pin-up" calendar for the Atlanta Dymes in 2005. In 2009, Williams met football player Kordell Stewart. The couple married in May 2011, which was shot and broadcast for the WE tv television series Platinum Weddings. She became the stepmother to his minor-aged son Syre from Tania Richardson's previous union.

Williams and Kenya Moore were cast for the fifth season of the reality television show The Real Housewives of Atlanta, alongside returning cast members Cynthia Bailey, Kandi Burruss, NeNe Leakes, Phaedra Parks, and Kim Zolciak. Williams made her first appearance of the season on November 18, 2012, where she would clash with Moore during a charity function for Feed the Homeless. The remainder of the season chronicled Williams' continuing feud with Bailey, as well as a later clash with Bailey after she felt that Williams lacked commitment to a pageant Bailey coordinator for her modeling company.

She also stated the intention to have children with Stewart, but she later saw a therapist after they began experiencing marital difficulties. Stewart filed for divorce from Williams in March 2013, after nearly two years of marriage. She proclaimed that she was "completely blindsided" by the filing and claimed she learned of their divorce via Twitter during the fifth season of The Real Housewives of Atlanta. Williams' divorce was finalized in December 2013; Williams received no money or earnings from the sale; he did not receive any of his assets or income.

In November, the sixth season of The Real Housewives of Atlanta premiered, chronicling Williams' marriage with her husband. During the season, she was briefly hospitalized, which the series portrayed as a result of her then-ongoing divorce. Williams received backlash after making remarks in which she said that the Underground Railroad was a legitimate railroad line following an episode of The Real Housewives of Atlanta's broadcast in December 2013. "It was a brain lapse," she later remarked. I have Porsha moments that you may not have experienced on the show before. I knew that I was supposed to go back and read up on my family's past, so I'm representing my legacy well." She appeared in Burruss' stage musical A Mother's Love.

On March 17, 2014, she unveiled her debut single "Flatline" on iTunes. Williams received media attention after an altercation with Moore while filming the sixth season reunion for The Real Housewives of Atlanta. Moore argued that Williams was unfaithful during her marriage; after Williams called Moore a "slut from the 1990s" and Moore yelled that Williams was "a dumb ho" from a megaphone, Williams assaulted Moore during the reunion special at the Biltmore Hotel on March 27, Moore said Williams was "a dumb ho" from a megaphone. Williams and McKinley split in June for a short time, but they have reunited as of August.

Williams left the Real Housewives of Atlanta after its thirteenth season in 2021 to star in Porsha's Family Matters, a spin-off series that is now in its first season. Williams' time on the Real Housewives earned her a devoted following; her story arc, which chronicled nearly ten years of Williams' life, is considered one of the franchise's finest. Fans and critics alike expressed dissatisfaction with Williams' departure. "Porsha's ability to weather the storm, as well as her personal growth, made her one of the city's most vivacious and likable Housewives.

Since George Floyd's assassination, Williams has worked as an activist and has been described by the New York Times as "a crusader against police violence." Williams was one of 87 demonstrators arrested outside of Kentucky Attorney General Daniel Cameron's home on July 15, 2020. She had been urging Cameron to arrest the officers involved in Breonna Taylor's shooting. Williams and others in the organization were charged with threatening a participant in a legal process, disorderly conduct, and criminal trespass after law enforcement warned that remaining on Cameron's property was unlawful.
