In 1975, Bolton began recording at The Church Studio in Tulsa, Oklahoma. Bolotin, his original family name, was used in this first album. Early in his musical career, he concentrated on hard rock, with his band Blackjack opening for heavy metal artist Ozzy Osbourne on tour. Bolton auditioned for, but was turned down, the lead vocalist position with Osbourne's former band, Black Sabbath, in 1983. "It's rumors that I auditioned for Black Sabbath was only a rumor," Bolton said later.

He became a big hit as a writer after anglicizing his family's name to Bolton, writing "How Am I Supposed to Live Without You" for Laura Branigan, who was once best known for singing the pop hit "Gloria." Branigan, who narrowly missed out on the top ten on the US pop chart, rose to the top of the Adult Contemporary chart for three weeks in 1983. Both men wanted to work together again, and they were able to co-wrote "I Found Someone" for Branigan in 1985. Cher's version was just a minor hit, but two years later, she resurrected the song, this time in the form of her first singing career. Both singers co-wrote several others. In 1988, Bolton performed his own version of "How Am I Supposed to Live Without You," a hit on the Hot 100.

Bolton's greatest success came in the late 1980s and early 1990s as a performer on the adult contemporary/easy listening charts. "Sittin' On) the Dock of the Bay" was one of his first major hits for Otis Redding's 1987 rendition. Zelma Redding, Redding's widow, said she was so moved by Bolton's appearance that "it brought tears to my eyes." It reminded me so much of my husband that I know if he heard it, he would feel the same." Bolton's popularity with that album prompted him to tackle the classic "Georgia on My Mind," which was his second hit. Bolton's 1991 release of Time, Love & Tenderness, which was his Grammy Award-winning cover version of "When a Man Loves a Woman."

Lucia Aliberti, Patti LaBelle, José Carreras, Tony Cetinski, Renée Fleming, Renée Judd, Wynonna Judd, BB King, Perpeto Sledge, and Zucchero have performed together as a singer. He has written and performed on several songs for other artists as well.

The 1997 hit "Go the Distance," which was included in the Disney animated motion picture Hercules), was Bolton's last Top 40 single in the United States in his own right, peaking at No. 22. Bolton appeared on The Lonely Island's "Jack Sparrow" as a guest vocalist on their Turtleneck & Chain album in 2011. His collaboration with the comedic hip-hop trio honed on his (intentionally) off-topic chorus and miscommunication with the group, and the video featured him dressed in costumes as Jack Sparrow of Pirates of the Caribbean, Forrest Gump, Erin Brockovich, and Tony Montana from Scarface.

Bolton was recording a new studio album as of February 2015. A. R. Rahman, an Indian musician, has collaborated on a piece that was released in February 2011 for Gems – The Duets Collection.

In 2013, Bolton released the album Ain't No Mountain High Enough: A Tribute to Hitsville, United States. It featured duets with Kelly Rowland, Melanie Fiona, and Orianthi.

Bolton appeared in Honda commercials in 2013 and 2014, in which he appears.

On the 11th season of Dancing with the Stars, Bolton was a contestant. On September 28, 2010, he and his dance partner Chelsie Hightower were the second couple to be eliminated.

While the family orders their pizza, Bolton appeared in a Pizza Hut commercial as himself singing Jingle Bells in 2015.
