Club career

Miazga, a student at Clifton High School in New Jersey, attended Clifton High School for one year before he left and joined the New York Red Bulls Academy in 2009 at a younger age. Miazga played for the Under-16 team in 2012 and helped the Red Bulls win the U.S. Soccer Development Academy National Championship.

Miazga had signed a letter of intent to play for the Michigan Wolverines men's soccer team in 2013. Miazga, however, turned down his scholarship offer to play in college and signed with the Red Bulls at the professional level, despite Europe and the New York Red Bulls' offer.

Miazga signed a homegrown deal with the New York Red Bulls on May 30, 2013, becoming the eighth player to sign a homegrown deal with the club. In the 76th minute of a 4–1 victory over Houston Dynamo on September 8, 2013, Miazga made his first-team debut at age 18 as a substitute for Markus Holgersson.

During the 2014 season, Miazga began playing more often. In addition, he signed an endorsement with Nike in August 2014 and appeared in the Major League Soccer Homegrown Game. Miazga's CONCACAF Champions League debut came on October 22 in a 1–1 draw against the Montreal Impact.

Miazga established himself as a starter for the team during the 2015 season, teaming up with Damien Perrinelle in the backline. In a 2–1 victory over New York City FC in the first ever Hudson River Derby match on May 10, he was suspended for two bookings. In a 3–1 victory over the same opponents at Yankee Stadium on June 28, he scored his first goal as a professional. He celebrated by pretending to swing a baseball bat, mocking the fact that their opponents play in a baseball stadium. Miazga was suspended for the second time of the season on September 25 in a 5–2 loss to Orlando City SC at the Red Bull Arena.

Miazga was regarded as one of the best defensive players in the league after solid results against David Villa of New York City FC and Toronto FC's Sebastian Giovinco, and the MLS team of the week award for four weeks during the season. New York defeated their second Supporters' Shield in three years by having one of the league's best defenses. Miazga signed a contract with the Red Bulls at the end of the season, making him one of the team's highest paid players. Miazga, on the other hand, turned down the bid, despite his inability to play in Europe.

Miazga joined Chelsea in England on a four-and-a-half year contract for £3.5 million (US$5 million). He was given the number 20 shirt for the remainder of the 2015–16 campaign three days later.

He was first called into a Chelsea matchday squad on February 16, but he was unused substitute in the first leg of the UEFA Champions League's 2–1 loss at Paris Saint-Germain. Miazga made his Chelsea debut in a 4–0 Premier League victory over Aston Villa on April 2, 2016, with both John Terry and Gary Cahill injured. Miazga started his second match against Swansea City on his debut but was forced to leave at halftime.

Miazga was loaned to Vitesse, a Dutch Eivisie club, for the 2016-2017 season after a contract with Espanyol fell through due to paperwork. In the Tweede Divisie's first round against SV Spakenburg on September 10, Miazga made his debut with a 2–2 draw. He was on the substitutes' bench against Ajax the next day, but he was an unused substitute as Vitesse lost 1–0. In a 2–1 away loss to Twente on September 25, he made his Educivisie debut on September 25, coming off the bench in the 64th minute and giving Lewis Baker an assist. Miazga scored his first and only goal for Vitesse in a KNVB Cup match against Jodan Boys, who lost 4–0.

He won the Cup with Vitesse in his first season with the club by beating AZ Alkmaar 2–0 in the final. He appeared in every match of the successful cup season.

Miazga's loan term at Vitesse was extended for the 2017–18 season on July 28, 2017.

Miazga joined Ligue 1 club Nantes on a season-long loan contract on August 6, 2018. Miazga made his Ligue 1 debut on August 18, 2018, starting and playing 90 minutes in a 2–0 loss to Dijon FCO.

Since returning from Nantes, Miazga joined Reading in the Championship club Reading until the end of the season. Reading decided to loan him out for the 2019-20 season after a fruitful loan experience. Miazga scored his first goal for Reading in a 1–0 victory over Preston North End on October 19, who had scored his first goal for the team.

Miazga joined Belgian side Anderlecht on loan until the end of the 2020-21 season on October 3, 2020.

Miazga played for the Spanish side Alavés from August 20, 2021, until the end of the 2021–22 season.

Miazga had returned to the United States, signing with MLS side FC Cincinnati on a three-and-a-half year contract on August 5, 2022.

International career

Miazga has represented the United States at under-14, 15, 18, 20 and 23 different levels around the world. In 2012, he appeared in one match for the Poland under-18 team. Miazga competed for the United States under the age of 18 in Portugal in May 2013.

Miazga's call-ups to the Poland under 20 team were turned down in August and September 2014. He cited previous commitments to the United States under 20 program while still deciding whether or not they should represent at the highest level. In October 2014, Poland U-20 coach Marcin Dorna said he had not given up on Miazga and that the Red Bulls player would still play for his team. Miazga competed for the United States under-20 team in the 2015 CONCACAF U-20 Championships in New Zealand and helped the team qualify for the 2015 FIFA U-20 World Cup. Miazga was a top performer at the 2015 U-20 World Cup, in which the Americans lost in the quarter finals to eventual champions Serbia on penalties.

Miazga was quoted as saying, "I'm entirely undecided" on November 5, 2015, a tidal greeting. I'll wait to see who calls me first to the main national team first and see what happens after." He was given his first call up to the United States for the 2018 FIFA World Cup qualifiers matches. In a 6–1 victory at Busch Stadium in St. Louis on November 13, 2015, he earned his first senior cap against Saint Vincent and the Grenadines. Miazga scored the Americans' third goal in a 3–0 win over Nicaragua in the 2017 CONCACAF Gold Cup, which took place on July 15, 2017.
