Milecofsky was born in New Jersey on August 29, 1972. In suburban Lakewood, New Jersey, where his father was a pharmacist and his mother was a real estate broker, he was brought up with his twin Marci and an older sister, Shari. Milecofsky legally changed his name to Ecko in 1996; he had intended to use "Echo," a word he was not familiar with at home, but he later decided on the alternate spelling because the word "Echo" was already taken.
Ecko enrolled in Rutgers University's School of Pharmacy in New Brunswick, New Jersey, after high school. During his time in college, he drew graffiti and sketched, adopting the term "Ecko" for his name. Ecko was encouraged to take a year off and explore art in his third year.

The company branched out into hip-hop and skater styles, and the rhinoceros logo was introduced.
