Fletcher appeared at the Anna Scher Theatre. In 1976, Bugsy Malone's first film role was as Baby Face in Bugsy Malone (1976). In A Midsummer Night's Dream's production, he made his stage debut the following year. He appeared in British films in the early 1980s, including The Long Good Friday, The Elephant Man, and The Bounty. In 1987, Fletcher was cast in Lionheart. Spike Thomson, the troubled teenager in Press Gang and Murder Most Horrid (1991) with Dawn French as an adult, appeared on television as the rebellious teenager Spike Thomson, as an adult. He has appeared in films Caravaggio (1986), The Rachel Papers (1989), Lock, Stock, and Two Smoking Barrels (1999), Layer Cake (2004), AffirmFilm's Solomon as Rehoam and Universal's Doom.

Fletcher has appeared in the major HBO drama, Band of Brothers, and in a supporting role in the BBC One historical drama The Virgin Queen (USA PBS 2005, UK 2006). In 1997, Kylie Minogue appeared in "Some Kind of Bliss" (1998), a video. He appeared on BBC One in a series based on Imogen Edwards-Jones' book Hotel Babylon, which ran for four seasons before being cancelled in 2009. He appeared in "The Booby and the Beast," an episode of the BBC's second series Robin Hood's series "Rob Hood" and the 2008 radio series The Way We Live Right Now.

He narrated the BBC1 show Del Boys and Dealers in 2014. Fletcher appeared on the album "Here Comes the Flood," which was released by The Divine Comedy in 1998.

Fletcher's debut as a producer on a script he co-wrote, Wild Bill, was published on March 20, 2012. Stephen Greenhorn's second film as director is Sunshine on Leith, which was based on the famous Proclaimers songs that were released on October 4, 2013. Eddie the Eagle, he's been in charge of the animated film Eddie the Eagle, in 2015. Fletcher was named as Bryan Singer's replacement director on the Queen biopic Bohemian Rhapsody on December 6, 2017. The film was released on November 2nd, 2018. Although Fletcher was assisting with the production, Singer received sole director credit and executive producer credit. Fletcher's most recent directing role was on Rocketman, a biopic about Elton John's life.
