When Swash was seven years old, he made his first television appearance in an Andrex commercial. In 1991, he was 9 years old and he was playing the role of a little boy pretending to be an orphanage in You Rang M'Lord. He was first cast in the film The Adventures of Pinocchio as The Battle Boy, starring Dawn French and Griff Rhys Jones and co-starring Jonathan Taylor Thomas, Richard Claxton, and Corrier. After going on a Roller Coaster, his character, who scuffled with Lampwick over a pistol, was turned into a donkey. He appeared in a few television series, including Casualty, The Bill, The Soldier," "Soldier Soldier" and Nickelodeon's LOL: Laugh Out Loud. Joe Swash appeared in the ITV documentary Club Reps. in 2002, he appeared as a participant.

When Swash was accepted as a member of BBC One soap opera EastEnders, he was going to give up on his dream of becoming an actor. He was portrayed as Mickey Miller in his first appearance in EastEnders in April 2003 as a recurring character. Producer Louise Berridge turned Mickey into a regular character, and his on-screen family arrived a year later, including Mickey's little sister Demi, who was played by Swash's real sister Shana. Harry Swash's son, Harry Swash, appeared on the soap as his niece Summer Swann for a brief period. Joe Swash and the actor who plays his stepfather, David Spinx, were both written out of EastEnders in a "very controversial storyline" on February 26, 2008. In July 2008, he made his last appearance. Swash has been critical of the direction producers took with his character since learning of his departure. Mickey's three-month absence in 2005 has been attributed to him becoming less than "light-relief."

However, he has expressed an interest in returning. Mickey Miller resurfaced his role on television for two episodes on September 19 and 20, 2011, in order to coincide with his on-screen brother's departure from the soap.

Swash appeared in the eighth series of the British reality show I'm a Celebrity...Get Me Out of Here! in November 2008, he went on to win.

Swash has been part of the presenting team on the spin-off film I'm a Celebrity from 2009 to present day. Get Me Out of Here, NOW! Russell Kane (2009–2010), Barbara Whitmore (2011–2015), and David Morgan (2015) co-presented the show with Caroline Flack (2009–2010).

I'm a Celebrity: Extra Camp was renamed in 2016 and the program was renamed I'm a Celebrity: Extra Camp. Vicky Pattison (2016), Stacey Solomon (2016), Chris Ramsey (2016), Joel Dommett (2017–2018) and Scarlett Moffatt (2017–2018). Swash had left the show and would not return for the season in 2019, according to the station's on July 20, 2019.

Swash appeared on Dancing on Ice in January 2020 as a contestant in the twelfth series. From week one until she sustained an injury in week 3, Swash was partnered with Alexandra Schauman. In week 4, he was re-partnered with Alex Murphy. Swash and Murphy were able to win the series after defeating Perri Kiely and his partner Vanessa Bauer.

Swash and ex-rugby player Austin Healey appeared in the second series of Hole in the Wall as a team captain in 2009.

He was a team captain on What Do Kids Know in 2010. On Watch, Rufus Hound and Sara Cox.

On the CBBC Channel in 2011, Swash introduced Gimme A Break. Swash and Caroline Flack, 2011-born on ITV2's game show Minute to Win It as team captains. Swash as the presenter/narrator of the CBBC Channel in November 2011 was broadcast on the CBBC Channel. Celebrity Driving Academy, a celebrity-driven academy, was broadcast in 2014 on television.

Swash was a regular team captain on ITV2's show Fake Reaction in 2013 and 2014. He appeared on The Jump as a contestant in 2016.

Swash appeared on the cooking show Celebrity Masterchef in 2021 when he reached the final 3.

Swash appeared in the pantomime Cinderella in Chatham, Medway, later Swash performed as Buttons.

He appeared in the pantomime 'Snow White and the Seven Dwarfs' in Bristol in 2009 and 2010.

Tim Vincent, a Swash and TV presenter, set a Guinness World Record for throwing the most pancakes to a partner in February 2009 as part of Channel 4's The Paul O'Grady Show.

Former EastEnders star Joe, 42, took exception to the big changes and demanded: 'What have you done to my garage?' Stacey reassured Joe that she will keep one of the doors the same for him, but he was uncomforted. Joe told Stacey: 'The proportions of this garage is the proportions of how our relationship works.
