Born in Tiel, Pieters, he grew up in Enspijk and began as a central midfielder in the junior ranks of VV Rhelico in Rumpt. He was scouted by FC Utrecht, and after a trial period, he joined the Utrecht youth academy.

Pieters made his Ethical debut for Utrecht in a 2–1 loss over Willem II and then went on to win in the Intertoto Cup playoffs for his second season. Pieters made his European debut in the Intertoto Cup in July of 2007. Utrecht finished in 10th place after he appeared in 35 games and scored twice against Willem II and FC Groningen. PSV Eindhoven's admiration for his solid performances attracted the attention of the team.

PSV Eindhoven, the European champions of sport, had signed the left back for reportedly €2.5 million. The number 14 shirt was given to pieters, which was previously worn by Slobodan Rajkovi. In a 5–1 victory, he made his PSV debut in the league against his former club. In September, he made his UEFA Champions League debut against Atlético Madrid. After a promising start to his PSV career, he was fired after a string of weak results. In 2008–09, the Pieters competed 23 times for PSV, finishing in 4th place and beating Feyenoord 2–0 in the 2008 Johan Cruijf Schaal.

Pieters recovered his position in the starting eleven and appeared on stage for the first two months leading up to the winter break. He was forced to return to within two weeks after suffering a knee injury while on international service. PSV finished in 3rd position and advanced to the round of 32 in the UEFA Europa League during the 2009-10 season. Pieters played 48 times in 2010-11 as PSV came close to winning the Dutch title, finishing third four points behind champions Ajax and fourth as PSV defeated Feynoord 10–0. Pieters suffered with a fractured metatarsal and played 25 times before deciding to have foot surgery.

After a nine-month layoff, he returned from injury in a league match against PEC Zwolle on January 18, 2013 and PSV lost 3-1 Pieters after a foul foul on Fred Benson. Such was Pieters' fury that he ran down the players' entrance and smashed a window, badly breaking his arm and extending his arm, and was banned from attending a further three months. He returned from the top of the table match against Ajax towards the end of the 2012–13 season, swapping Jetro Willems after 73 minutes. Derk Boerrigter scored the winning goal and Ajax won the Eredivise thanks to a mix-up between Pieters and PSV goalkeeper Boy Waterman.

On June 28, 2013, Pieters agreed to Stoke City, an English club, for a four-year deal on a four-year deal, worth €3.6 million (£3 million). Pieters admitted that his main aim was to play in England after deciding to join Stoke. "Of course, I've always wanted to play in the Premier League." It's one of the most difficult tournaments to participate in with some of the best players and teams, and I'm really excited about it. I'm really looking forward to playing for this club. For me, it's important that the first impression is positive, and Stoke City has a great reputation. In a 1–0 loss to Liverpool on August 17, he made his Stoke debut on September 17th. Pieters scored his first goal for Stoke on April 12, 2014, after his intended cross beat Tim Krul. In 2013, the Pieters played 39 times for Stoke as they finished in ninth place in the Premier League.

Pieters remained in second place in 2014-2015, having played in 32 games as Stoke finished in ninth position. Stoke suffered with a groin injury throughout the season. Stoke beat Liverpool 6–1 to bring an end to the season. Pieters signed a two-year contract extension in December 2015, extending his stay at the club until 2020. Pieters appeared 41 times for Stoke in 2015–16, when the Potters finished ninth in the Potters' fifth place. Pieters made more tackles than any other defender in the Premier League during the season, according to Opta Sports.

As Stoke finished in 13th place, the Pieters remained Hughes' first choice left back in 2016-2017, racking 38 appearances. Pieters were chastised at times during the campaign, but assistant manager Mark Bowen admitted that Pieters "has come off his standards." After attending a late night dance just hours before a league match against Everton, Pieters was fined £70,000 by manager Paul Lambert in March 2018. Pieters paid for Burnley for an undisclosed fee on August 8, 2019. Burnley's Joe Burnley announced on June 10, 2022, that Pieters would leave the club at the end of June after his deal came to an end.

Pieters joined West Bromwich Albion on a free transfer on September 13, 2022, extending their contract until the end of the season. In a home loss to Birmingham City, he made his Albion debut the following day and was deployed as a substitute.

International career

Pieters appeared in five matches for the Netherlands at the 2005 FIFA U-17 World Championship, where they advanced to the semi-finals. In 2007, Pieters was called up by Netherlands U-21 coach Foppe de Haan to be part of his team for the 2007 European Under-21 Football Championship, held in the Netherlands. Pieters did not participate in their first round group match against Israel (1–0 win), but they did come on as a replacement in their second match against Portugal (2–1 victory) and qualify for the 2008 Summer Olympics.

Pieters started in the starting eleven against England (1–1, 13–12 after 32 penalty kicks), but Tim Janssen later changed them to enhance the offense as well as due to a small injury. Pieters returned to the field, and the Dutch won their 2006 title by defeating Serbia 4–1 in the final. Pieters was included in the provisional list of 36 players for the Euro 2012 tournament on May 7, 2012, but he did not recover in time for the tournament due to a foot injury. Jetro Willems, a member of PSV, was swapped for him in the Euro 2012 squad.
