In the 1980s, Cox worked for Bethesda Softworks for a short time. As the young woman pulled onstage at the St. Paul Civic Center to dance with Springsteen, she was chosen from a casting call by director Brian De Palma to appear in Bruce Springsteen's "Dancing in the Dark" 1984 music video. She Wrote (1986) and the CBS crime drama series Murder include a starring role as Gloria Dinallo in the short-lived NBC science-fiction fantasy series Misfits of Science (1985) and guest-starring roles in the ABC comedy-drama series Misfits of Science (1986). Lauren Miller, Michael J.'s girlfriend, had a recurring role as Lauren Miller later in life. Alex P. Keaton of Fox in the NBC comedy film Family Ties (1987–1989).

Cox's early film appearances include Masters of the Universe (1987), Cocoon: The Return (1988), and I'll Be Home for Christmas (1988). In Mr. Destiny (1990), she appeared in Jewel Jagger, Larry Burrows' tough-as-nails assistant (James Belushi). She appeared on the short-lived CBS sitcom The Trouble With Larry, alongside Bronson Pinchot and Perry King, in 1993. Meryl, Jerry Seinfeld's girlfriend, appeared alongside Jim Cary in the comedy film Ace Ventura: Pet Detective" and also in the Seinfeld episode "The Wife" earlier this year. Cox was invited to audition for Rachel Green's lead role in 1994's Friends, but she was replaced by Monica Geller.

Cox appeared on "Rachel Green," Jennifer Aniston (Phoebe Buffay), Matthew LeBlanc (Joebbiani), Matthew Perry (Chandler Bing), and David Schwimmer (Ross Geller) for what became her most well-known role on the new show, lasting for ten seasons until 2004. She was first, the most well-known cast member of the show. The series is generally regarded as one of the best sitcoms of all time. She began acting as the star of ABC comedy show Cougar Town in 2009, portraying a young single 40-year-old mother on the hunt for new experiences. It is notably Cox's most popular work since Friends that has received a nomination for Best Actress – Television Series Musical or Comedy at the Golden Globe Award. In November 2011, the series's third season was supposed to premiere in November 2011, but it was delayed to February 14, 2012. Two episodes of the series's fifteen episodes were produced by Cox. On January 8, 2013, the fourth season premiered. After six seasons, the series came to an end on March 31, 2015.

Gale Weathers from the Scream trilogy appeared in Cox's sequel Scream 4 as Gale Weathers. On April 15, 2011, the film was released in theaters.

Cox made her directorial debut and appeared in a Lifetime television drama film TalhotBlond, which premiered on the network on June 23, 2012. Cox produced and produced Just Before You Go, starring Seann William Scott and Elisha Cuthbert, in 2014.
