Coaching career

McDermott joined the Philadelphia Eagles in 1999 as a Scout administrative coordinator, a position he held until being promoted to Andy Reid's coaching staff in 2001. He then became a defensive quality control coach and later assistant defensive backs coach. As Spagnuolo's first linebackers coach, he named Steve Spagnuolo as the defensive backs coach in 2003. For the first time in team history, McDermott and his starting safeties (Brian Dawkins and Michael Lewis) earned Pro Bowl berths. McDermott and the Eagles appeared in Super Bowl XXXXXXXXIX last season, but the New England Patriots lost their second straight Super Bowl title to the New England Patriots in Super Bowl XXXIX. Following McDermott's tenure as a coach, the Dawkins earned two more Pro Bowl berths in 2005 and 2006.

After Spagnuolo had to leave to take the defensive coordinator position for the New York Giants in 2007, McDermott was assigned linebackers coach. McDermott was named as the secondary coach again by Eagles head coach Andy Reid on January 28, 2008.

McDermott, the interim defensive coordinator, was appointed interim defensive coordinator as a result of defensive coordinator Jim Johnson's medical leave of absence on May 18, 2009. The Eagles announced McDermott would take over as full-time defensive coordinator on July 24, of the same year, as a result of Johnson's continuing decline. Johnson died just four days later. McDermott will go on to develop a variety of blitzes in his later defensive gameplans thanks in large part to what he learned under Johnson.

After 12 years with the Eagles, McDermott was fired as the defensive coordinator on January 15, 2011.

On January 17, 2011, McDermott was hired as the Carolina Panthers' defensive coordinator. McDermott was reunited with new Panthers head coach Ron Rivera, a former Eagles assistant with whom McDermott worked alongside from 1999-2003. In 2015, McDermott became Pro Football Focus' second runner up to their Defensive Coordinator of the Year award.

McDermott, the Panthers' defensive coordinator, led the team to finish in the top ten in overall defense from 2012-2015.

McDermott and the Panthers reached Super Bowl 50 in 2015, which was played on February 7, 2016. In the game, his defense only gave up one offensive touchdown, but the Panthers lost by a margin of 24–10.

McDermott was hired by the Buffalo Bills as the 19th head coach in franchise history on January 11, 2017.

McDermott won his NFL head coaching debut in the season's opening 21–12 victory over the New York Jets on September 10, 2017, becoming only the third Bills head coach to win his first game with the team after Marv Levy and Rex Ryan.

McDermott will lead the Bills to 4 victories in the next five games, including a victory over the reigning NFC champion Atlanta Falcons following a Week 2 loss to the Carolina Panthers and McDermott's first return to Charlotte after leaving the Panthers organization. However, the New Orleans Saints lost by 47–10 to the New Orleans Saints, which prompted him to make the controversial call to bench starting quarterback Tyrod Taylor in favour of rookie backup Nathan Peterman. Peterman failed against the Los Angeles Chargers in his first attempt at his career in his first appearance, throwing 5 interceptions in the first half. During the second half of the 54–24 loss, which reduced the Bills to 5–5, Taylor was benched for Taylor.

Despite a string of losses, the Bills went on to finish the season at 9–7, clinching the AFC's 6th seed and first playoff appearance in 18 years, bringing an end to both the NFL's and the North American professional sports franchise's longest active playoff droughts began during McDermott's first year as head coach. In the AFC wild card game, the Bills will lose 10-3 to the Jacksonville Jaguars.

McDermott's Bills went 6-10 and missed the playoffs this season, but the Bills still had a good start to the season after a 2–7 start. The Bills changed their roster after losing in four of the first nine games, partially due to a lack of offensive talent. In 2018, Buffalo's defense was updated.

After leading the Bills to a 10–6 record and their second playoff appearance in three seasons as the AFC's No. 5 seed, McDermott was nominated for NFL Head Coach of the Year for the 2019 season. During overtime, the Bills will lose 22–19 to the Houston Texans.

McDermott renewed his contract until 2025 on August 12, 2020.

McDermott and the Bills saw a number of instances of growth and success in the 2020 season. Quarterback Josh Allen made a name for himself by turning into an MVP nominee and leading the Bills to their first AFC East Division title since 1995, as well as a 13–3 record, tied for second best in the league with the Green Bay Packers and behind the Kansas City Chiefs. They also tied for the most games set in 1990 and 1991. The Bills reached their high point in 2020 with a cumulative record of 15–4.

The Bills increased their liability against Quarterback Josh Allen by drafting Offensive Lineman Spencer Brown from Northern Iowa prior to the 2021 season. The Bills went on a four-game winning streak after losing their first game against the Steelers, 23-16, including two shutout victories against the Dolphins (35-0). The Texans (40-0) defeated the Dolphins (36-0). The Bills will continue to dominate their division finishing 5-1, with the only loss coming against the Patriots in a game with 25-35 mph winds. Overall, the Bills will finish 11-6. They will face the Patriots 47-17 in an AFC Wildcard matchup. There was a lot of anticipation going into the AFC divisional matchup, as they face the Kansas City Chiefs, who had lost to the year before. The Bills will lead in game 36-33 in what is regarded as the best game of the 2021-2022 season, with :13 seconds remaining in the 4th quarter. Since a total of 25 points were scored in the final 1:57 of the game, overtime would begin, bringing the total score to 36-36. To win the game 42-36, the Chiefs would win the opening OT coin toss and then run down the field.
