Agyeman began her professional career by using Frema, a different spelling of her birth name, to avoid pronunciation issues.

Agyeman's most well-known television role in the revivaled series of the ITV soap opera Crossroads, Martha Jones was playing Lola Wise before securing the role in Doctor Who. At the 2003 British Soap Awards, she was nominated in two categories, Best Newcomer and Sexiest Female. Agyeman appeared in small guest roles in such television series as BBC's long-running medical drama Casualty, Sky One's airline drama Mile High, and ITV's police procedural The Bill, in which she appeared twice as two separate characters. In an episode of Silent Witness, she appeared Mary Ogden, a scene of crime officer.

Agyeman made her debut in 2006, playing Nana in the independent drama Rulers and Dealers (2006), written and directed by Stephen Lloyd Jackson.

In the 2006 film Doctor Who, Agyeman auditioned for three parts. She auditioned for the role of Sally Jacobs in "The Christmas Invasion" on June 24, 2005, but the actress was later dismissed by Anita Briem, who more accurately represented the character as a "ice cool blonde." In the "Rise of the Cybermen"/"The Age of Steel" and "Army of Ghosts," respectively, Agyeman auditioned for Esme and Adeola Oshodi. Esme was eventually dropped from the final script, but Agyeman's audition for Adeola was fruitful. She filmed her role in the series in December 2005 and then appeared on television as Adeola on July 1, 2006. Agyeman studied dance and practiced horse riding, martial arts, and gymnastics, a fact that the designers exploited in order to provide her with more physically demanding action scenes.

Agyeman's versatility during three auditions was admired by the production team, who has recommended her as a good candidate for the new companion. She appeared in another audition (ostensibly for an episode of Torchwood) before heading to her final screen test with Tenth Doctor actor David Tennant. Tennant left a reassuring note under the door of Agyeman's hotel room, which helped to sooth any nerves. Agyeman was announced to the press as the new companion Martha Jones on July 5 following extensive rumors over who would replace Billie Piper.

Agyeman began filming for his third series in August 2006 and ended in March 2007. Martha made her film debut as a medical student on the episode "Smith and Jones" on March 31, 2007. Adeola was Martha's cousin, according to a line of dialogue. Agyeman portrayed the character in every episode of the series. Martha left the Doctor of the Time Lords of the series finale after noting how bad their relationship had become, but she promised she'd see him again. Agyeman appeared in the animated series The Infinite Quest, which aired in twelve weekly segments during the 2007 series's run.

Several award ceremonies were lauded Martha Agyeman's performance in 2007. She was named Best Newcomer at the Glamour's Women of the Year Awards in June. She and several other former cast members followed David Tennant's last appearance in "The End of Time" later in the series. Agyeman read abridged audiobook versions of five of Martha's Doctor Who New Series Adventures books, including The Last Dodo, Wetworld, The Pirate Loop, Martha in the Mirror, and The Story of Martha.

Agyeman appeared in three episodes of Doctor Who's spin-off drama Torchwood, including "Reset," "Dead Man Walking" and "A Day in the Death" in between the third and fourth series. She appeared in the Torchwood radio play "Lost Souls" on BBC Radio 4 on September 10, 2008, to announce the activation of the Large Hadron Collider at the CERN research center on the same day. The production team had intended for Agyeman to appear in the third series of Torchwood, but she had already committed to Law & Order: United Kingdom, which was later revealed by director Euros Lyn.

She reprised her role as Martha in the Torchwood audio drama "Dissected" produced by Big Finish Productions in February 2020. The Year of Martha Jones, a three-part audio series starring Agyeman, was announced in July 2021. In the third series finale of Doctor Who, it was announced in December and depicted Martha during the year she spent on the Earth in the year she spent travelling the Earth.

Agyeman appeared on CBeebies' The Bedtime Hour several times, telling the tale shortly before 7 p.m. She narrated the first series of BBC Three's hospital documentary series Bizarre ER, which ran from 14 February to 3 April 2008 and also provided the narration for a second series that premiered on April 21, 2009. On July 27, Agyeman hosted a Doctor Who-themed portion of the BBC Proms. On BBC One, New Year's Day, 2009, a one-hour cut-down version of the prom made its television debut.

In the Emmy Award-winning 2008 BBC adaptation of Charles Dickens' Little Dorrit, Agyeman appeared alongside her Torchwood co-stars Eve Myles and Ruth Jones. Some of Agyeman's scenes from the first episode were shot on the grounds of Deal Castle, Kent, which doubled for Marseille, France. Agyeman was thrilled to have appeared in Little Dorrit, as it had always been a dream for her to appear in a period drama. Agyeman appeared in Survivors, a BBC One remake of the 1960s television series, as Jenny Walsh, although her character was not revealed to be Collins) in the first episode in a surprise twist despite prominently in promotional literature for the series.

Crown Prosecutor Alesha Phillips appeared in Law & Order: UK, a UK adaptation and spin-off from the US Law & Order franchise, between 2009 and 2012. Agyeman is a self-professed "massive fan" of the original Law & Order film, especially the third and fourth seasons. Agyeman sat on live trials, toured the Old Bailey, and talked with her law-degree holder Leila: "She's amazing and really patient in explaining stuff to laymen because I was going into it entirely blind." She's been a reliable source." She stayed with the program for the first six series before the seventh series due to other filming commitments. Agyeman was longlisted for the National Television Awards for Outstanding Drama Performance several times for her role as Alesha Phillips. She revealed in July 2022 that she would not be returning to New Amsterdam for its fifth and final season, instead choosing Dreamland, which will star singer Lily Allen in her first television lead role.
