Lewis was born in Salem, Michigan, United States.

She was writing and recording by her teens with record producer Ollie McLaughlin, a black DJ at Ann Arbor radio station WHRV, now WAAM. Lewis' first single release, "My Heart Went Do Dat Da" in 1962, did not chart nationally, but it was a local hit in the Detroit, Michigan area. She wrote all of the songs on her debut album, including the hit "Hello Stranger" which debuted at No. 1 in the United States. The Hammond organ was featured on the US Billboard Hot 100 chart, at number 3, and it made extensive use of the Hammond organ. Lewis had moderate success with "Straighten Up Your Heart" (#43) and her original "Puppy Love" (#38) before Bert Berns introduced her million-seller "Baby I'm Yours" (U.S. #11), written by Van McCoy. Berns also produced "Make Me Your Baby" (U.S. #11), which had been released by the Pixies Three, and Lewis' final Top 40 hit "Make Me Belong to You" (#28 in 1966), written by Chip Taylor and Billy Vera.

She released a grittier-sounding album on Stax Records at the end of the decade.

A number of other musicians have had success with Lewis' songs over the past decade. In 1973, Elliman's composition "Hello Stranger" — which had been remade by Fire & Rain in 1966 — became a regional hit, and her version topped the US Easy Listening chart for four weeks. Carrie Lucas's "Hello Stranger" was a Top 20 R&B hit in 1985, and Queen Latifah remade "Hello Stranger" for her The Dana Owens Album in 2004.

Lewis had been out of public view for years after her career slowed in the 1960s. It was only after Elliman's 1979 debut that Casey Kasem tracked her down for his AT40 show on June 4 of that year. Nobody knew where she had ended up, including her handler, who didn't even know where to send her checks for the Elliman case, according to Kasem. When she was discovered in Michigan, she was wishing to be rediscovered, according to Kasem.

Lewis had to stop performing in 2017 due to health problems.

"Baby I'm Yours" charted in versions by country singer Jody Miller and Debby Boone (the B-side of her single "God Knows"). Suzanne Stevens, a Canadian singer, had a success with a disco version of "Make Me Your Baby" in 1975. The Arctic Monkeys' version of "Baby I'm Yours" as a B-side to their 2006 single "Leave Before the Lights Come On," continuing with the latest millennium's coverage.

Lewis' "Baby I'm Yours" appeared on the soundtrack for the film The Bridges of Madison County in 1995, and "Hello Stranger" was included on the soundtrack for the film Moonlight in 2016. In 2019, "Hello Stranger" was shown at the end of the British TV series Giri/Haji's final episode.

In 1999, she received the Rhythm and Blues Foundation's Pioneer Award. Barbara Lewis was inducted into the Michigan Rock and Roll Legends Hall of Fame in 2016.
