In 1975 satire Smile, O'Toole's first big film appearance was as a jaded beauty pageant contestant; she was given the role after doing an impression of a "dead cockroach" at the audition. In 1977, she appeared as the tutor and girlfriend of Robby Benson's character in the college basketball thriller One on One. In the 1980 film Foolin' Around, she co-starred opposite Gary Busey. She appeared in Vanities' onstage production as well as in the television documentary Stand By Your Man, which chronicled Tammy Wynette's life. In 1982, she appeared in 48 Hrs as Nick Nolte's girlfriend for a brief period of time. She appeared in Cat People in 1986, later in 1983 she appeared Lana Lang (love interest to Clark Kent/Superman) and Ricky in Superman III.

She costarred with Barry Manilow in the CBS television film Copacabana, starring Lola La Mar to Manilow's Tony Starr in 1985. She appeared in the original Bridge to Terabithia in 1985 and 1985, and she also appeared in Strong Medicine's television version the following year. O'Toole, a romantic comedy in 1987, had a leading role opposite Martin Short.

O'Toole appeared in two ABC television mini-series in 1990. So she slowed down to drive a rented car. On the long ride up there, the two of us assembled together, and she told me she had a tune in her head." The "tune in her head" became "Potato's in the Paddy Wagon," one of three songs written by the couple for A Mighty Wind, including the Oscar-nominated song "A Kiss at the End of the Rainbow."

O'Toole sang "What Could Be Better?

"for the 2004 Disney children's album A World of Happiness, she and her husband co-wrote a song for the Disney children's album A World of Happiness."
