The Iowa native was a lunar module pilot on the Apollo 7 mission in 1968, the first successful crewed flight of NASA's Apollo program. NASA announced his death but did not specify the reason, although a family statement said he died in hospital 'from the effects of a fall' after a long and healthy life.' He was pictured left during the Apollo 7 mission in October 1968 and then on to top right in October 2014.
