Early life and education
Joan Smalls Rodrez was born and raised in Hatillo, Puerto Rico, with her two siblings. Eric Smalls, her father, is an accountant and comes from Saint Thomas, and she is a West Indian of African and Irish origins. Betzaida Rodrón, the child's mother, is a social worker who lives in Puerto Rico and has a Spanish and Tano roots. She also has distant Irish and Indian ancestry. Smalls were initially interested in modeling as a youth and then began to compete in local competitions. During this time, she was working with a New York company that told her she would have to correct her teeth in order to succeed; during that time, she went to college. Smalls graduated magna cum from the Interamerican University of Puerto Rico with a bachelor's degree in psychology. She then moved to New York City to begin her modeling career after graduating.

She was recruited by Elite Model Management in 2007 and worked in commercial modeling positions such as Nordstrom, Liz Claiborne, and Sass & bide. She left Elite and joined IMG Models around the world in 2009, where she shifted her attention to runway work. She was selected as a Givenchy exclusive for Riccardo Tisci's Spring/Summer Haute Couture show in January 2010, and her modeling career gained traction.

In late 2017, Jeremy Scott, the creative director of the fashion house Moschino, unveiled Joan Smalls as the new star of the new Moschino fragrance, Fresh Couture Gold, shot by Giampaolo Sgura. This was Joan Smalls' first fragrance launch.
