Professional career

In the seventh round of the 2009 Major League Baseball draft, the San Diego Padres selected Mikolas.

In 2010, Mikolas pitched as a reliever for the Class-A Fort Wayne TinCaps, finishing 2nd in the Midwest League) and posting a 2.20 earned run average (ERA) in 60 games (leading the league). He was named a Midwest All Star.

He started with the High-A Lake Elsinore Storm in 2011 and was promoted to the Double-A San Antonio Missions on July 4 after going 3–0 with 12 saves (4th in the California League) and a 1.13 ERA in 34 games. Mikolas eke out 2011 by going 1–0 for nine saves and compiling a 1.67 ERA as a relief for the Missions over 28 games.

In 2012, Mikolas was accepted by a non-roster invitation to spring training. He was scheduled to the youth league camp later in the spring and started the year with San Antonio. Mikolas was called up to the majors for the first time by working from Double-A San Antonio on May 5, 2012. He made his Petco Park debut against the Miami Marlins, for whom he grew up rooting. Giancarlo Stanton, the first batter he met, was on a home run. After making 13 appearances and allowing five earned runs in 1323 innings, Mikolas was sent down to the Triple-A Tucson Padres on June 5, allowing five earned runs in 132 runs. On July 5, he was recalled and made two more appearances before being recalled to Triple-A. He was recalled from Tucson on August 5 and stayed with the big league team through the rest of the year. Mikolas went 2-1-1 for the season, with 23 strikeouts against 15 walks over 3213 innings.

He was 4–2 with a 3.25 ERA and 26 saves (3rd in the Pacific Coast League) in 2013 with Tucson. For the Padres, he pitched 1+2/3 scoreless innings. Mikolas was slated for assignment by the Padres on November 20, 2013.

After the 2013 season, the Padres traded Mikolas and Jaff Decker to the Pittsburgh Pirates with Alex Dickerson. In exchange for Chris McGuiness, the Pirates traded him to the Texas Rangers during the offseason. He joined the Round Rock Express in 2014 and spent the first year as a coach.

On July 1, 2014, Mikolas was added to the Rangers' 25-man roster. He had a 5–1 win-loss record in 44+2/3 innings pitched prior to his promotion in a 5.22 ERA. Mikolas was playing against the Baltimore Orioles in his first game in his career, giving up three hits and three runs in 5+13 innings. He appeared in ten starts for the remainder of the season with Texas, going 2–5 for a 6.44 ERA in ten starts. Mikolas was cut by the Rangers after the 2014 season.

Mikolas signed a one-year, US$700,000 contract with the Yomiuri Giants of Nippon Professional Baseball on November 25, 2014. He was 13-33 years old in 2015, when he was 13–3. The Giants re-signed him to a two-year contract after the season. He made a 3.18 ERA in 62 starts in three seasons with Yomiuri.

On December 5, 2017, Mikolas agreed to a two-year, $15.5 million contract with the St. Louis Cardinals. He made his Cardinals debut 8–4 against the Milwaukee Brewers on April 2, 2018. While batting, he hit Zach Davies for his first major league hit. He hit out five runs and four runs on the mound, giving three home runs and four runs in total, with three home runs and four runs on the road.

On May 21, 2018, the Kansas City Royals threw his first major league complete game, a shutout, in Busch Stadium. As the Cardinals defeated Kansas City 6–0, he struck out nine, walked one, and gave up only four hits. Mikolas was named to the 2018 Major League Baseball All-Star Game, his first All-Star pick, with a 9–3 record, a 2.63 ERA, and a 1.03 WHIP.

Mikolas concluded his 2018 season with an 18–4 record (including a 10-0 record on the road), a league-leading.818 win-loss percentage, a 2.83 ERA (4th in the National League), and 29 batters in 29 innings (6th). For the NL lead, he and Jon Lester and Max Scherzer are tied for 18 wins. He led the major leagues with a 0.30 BB/9, and he also led all major league pitchers in first-strike percentages (70.8%). In addition, he had the highest zone percentage of all major league pitchers, with 48.0% of his pitches being in the strike zone. He was second among NL pitchers in range factor per 9 innings (2.20), third in putouts (19), and fourth in assists (30). In the 2018 NL Cy Young Award nominations, he came in fifth position.

Mikolas and the Cardinals decided on a four-year contract extension worth $68 million on February 26, 2019. He had a 9-14 record and a 4.16 ERA in the 2019 regular season, knocking out 144 out of 144 over 184 innings. He was named as the 2019 National League Division Series' Game 1 starter for St. Louis. He missed the entire 2020 season due to injury to a flexor tendon in his right arm.

Mikolas was activated from the 60-day injured list on August 20, 2021, after missing the previous 75 games due to a right forearm strain.

In a game against the Pittsburgh Pirates on June 14, 2022, Mikolas threw 8+2/3 no-hit innings. Cal Mitchell, Mikolas, was allowed a ground-rule double with two strikes in the count to the next batter.

After replacing Corbin Burnes, Mikolas was admitted to the 2022 Major League Baseball All-Star Game.

The 41-year-old MLB veteran had been expected to start the Cardinals' first game of the 2023-24 season on the mound but Miles Mikolas replaced him last week after suffering a groin injury last week. That didn't matter to the home crowd, though Cardinals fans were eager to have their 2006 World Series champion involved in what may have been their last opening Day. As he announced that a "up-and-coming singer" from St. Louis was taking center stage, fans went wild after learning it was the three-time All-Star.
