Early life
Polaha was born in Reno, Nevada, to Esther and Jerome Polaha. "Polaha is Czech," despite his Hawaiian-sounding last name. His father, who was a Washoe District Court Judge, was a judge. Polaha is the youngest of four brothers. In the early 1990s, he left Reno to attend Robert Louis Stevenson's boarding school in Pebble Beach, California. He graduated from New York University Tisch School of the Arts in 1999, where he obtained a Bachelor's degree in Arts. He moved to Los Angeles on July 4, 2001.

Career
Polaha appeared in theatre productions, including Ragtime Revue at Lincoln Center, The Long Days Journey Into Night at the Roundabout Theater, Bread & Butter at Provincetown Playhouse, and Uncle Vanya at the Stella Adler Conservatory early in his career. Polaha began to appear in small roles in several American television shows and films in the early 2000s. Gavin's first on-screen appearance was in the comedy-drama series That's Life, Episode "Plus One". He appeared in television series Angel, Roswell, Birds of Prey, Tru Calling, House, and Close to Home in Philadelphia, Bones and Close to Home. In the TBS film America's Prince: The John F. Polaha will appear in Hallmark Channel's Christmas-themed television film Double Holiday (2019) opposite Carly Pope on July 26, 2019. According to Variety, Polaha had signed a book contract with Rosewind Books to co-author a series of romance books with Anna Gomez in June 2020. Moments Like This, the first book in the From Kona with Love book series, is set in Hawaii and it follows a young woman struggling to find herself after losing her career and love. It was released on February 2, 2021, 2021. Where the Sun Rises, Vesuvian Media's Rosewind Books' second book, is set to be published on March 29, 2022.
Through his production company Podunk Productions, Polaha intends to produce the books for film and television.
