In the bottom of the third inning, Francisco Alvarez grounded out with two outs and a runner on first, ending the inning as Miami forced an out at second base. But Marlins second baseman Otto Lopez apparently didn't realize his team had reached three outs as he fired the ball towards first base after catching a throw from teammate Emmanuel Rivera.
