Education
Downs attended the University of Virginia on a volleyball scholarship after graduating from Pelham High School in 1998. She was an Echols Scholar and a member of the Jefferson Literary and Debating Society while attending the University of Virginia. Downs' freshman year at the University of Virginia, she decided to focus on her education and aspiration to attend medical school, ultimately moving to Samford University. She was denied her scholarship after her transfer, so she began competing in local pageants to earn scholarship funds. She earned a degree in history and a double-minor in biology and chemistry from Samford University in 2002. In 2002, she was also a finalist for the Rhodes Scholarship.
She began medical school at the University of Alabama School of Medicine (UAB) in Birmingham after completing her stint as Miss America. She earned her Doctor of Medicine in 2010 and then completed her residency in Obstetrics and gynecology at the UAB Medical Center in 2014. She continued her education at the University of California and completed her fellowship in reproductive endocrinology and infertility in 2017.

Deidre Downs Gunn, Downs' college sweetheart, was married to Andrew Gunn in March 2008. In 2010, they had a boy. The couple then divorced.
In 2018 at a private celebration at Birmingham's Museum of Art in Birmingham, Alabama, the Downs remarried. Abbott Jones, her wife, is both an attorney and writer. Downs is the first Miss America national champion to marry in a same-sex union.
