Tommy was born in County Durham as a youth, but as a teenager, he was banned at 15 years old and joined Hartlepool United, where he signed his first professional contract. He attracted the attention of several clubs and so left the club in 2001 after 160 appearances and 41 goals in Division Three. Miller was close to joining Norwegian side Brann in order to participate in the Champions League Campaign on a three-month loan contract before joining Ipswich Town. Miller's move to Brann was however unlikely, because FA rules state that a player can only play for a team outside of the United Kingdom for a minimum of three months.
On July 12, 2001, boss George Burley sold him back to Ipswich. Miller made eight league appearances in his first season with Ipswich Town, and his side was 18th in the Football League Division One, which resulted in his relegation to Football League Division One. Miller won in 3–1 over Swedish side Helsingborgs on November 1, 2001. Miller made his league debut against Middlesbrough on Friday, when he came on as a replacement on a 75-minute draw against Sixto Perpeta. Miller was involved in the First Round against Smederevo and won 2–1 on aggregate, despite being out in the UEFA Respect Fair Play rankings.
In the 2004–05 season, he spent four years at Ipswich, scoring 15 goals in 50 appearances and assisting them in an unsuccessful Championship play-off appearance. After Miller's last season, the team was offered a new deal for a two-year deal, but only for him to withdraw as his deal was due to end, which attracted players from Premier League and Championship clubs.
In June 2005, he came to Sunderland on a two-year deal, defeating Scottish Giant Celtic and Leeds United, who were chasing after him. Miller made his club debut against Charlton Athletic on August 13, 2005, beating them 3–1. Miller scored his first goal in the Premier League and Sunderland's 2-0 victory over Middlesbrough on Sunday, giving the Premier League their first victory since 2002. Miller scored his second goal in a 1-1 draw against West Ham United in October 2005. Miller lost by 2–1 against Portsmouth on April 22, 2006, but it took 7 months for him to score in a 2–1 loss. However, he failed as they returned to the Championship after only one season, finishing last with a new low of 15 points. Miller appeared in 29 games in the Premier League during his first season as a midfielder.
He appeared in the first three games of the season under new manager Niall Quinn, but after his appointment of Roy Keane he was left sidelined and in the pecking order that resulted in his loaning to Preston North End. Miller made his club debut against Coventry City on November 28, 2006, a 1–1 draw. Despite being on loan, Miller beat his parents club 1–0 on December 30, 2006. Miller made his last game for the club on March 10, 2007, defeating Barnsley 2–0. He was suspended at the end of the 2006-07 season after Sunderland were promoted to the Premier League for the second time following just one season at the Championship.
Miller had been linked to Championship sides Stoke, Coventry and Sheffield Wednesday, as well as SPL side Hearts, since being released. Steaua București, who were also keen on a move, was also interested in reporting that he might be negotiating a transfer. Miller, on the other hand, moved to Steaua București in order to remain in England.
Miller started his third stint with Ipswich Town on July 19, 2007, extending his deal with the Championship club to two years. In November 2006, Ipswich Town had been linked to a signed loan, but Preston decided against joining Preston. Miller won by 4–1 over Sheffield on August 11, 2007, his second appearance for the club. Miller failed to score many goals in the first six months, but he scored 3 in the last 6 and finished his season with a score of 5.
Miller and seven others were released by Ipswich on May 8, 2009.
"I have made it clear that we need to bring knowledge to our team," Miller said after finishing the signing of Miller for Sheffield Wednesday's manager Brian Laws. He also has the ability to carry out the experience and a tenacious desire to succeed. I am delighted to welcome him to the team. Miller has been in possession of a two-year deal. Sheffield Wednesday made an attempt for Miller in December 2001, but Ispwich Town refused to keep Miller because Manager Joe Royle wanted to keep Miller. Miller made his club debut against Peterborough United on August 15, 2009 in a 1–1 draw. In a 2–2 draw with Preston North End, he scored his first goal. After being fired, Brian Laws left the club and went on to face Burnley in the Premier League later this season, and Alan Irvine replaced him. In addition, Sheffield Wednesday was relegated to the last game of the season after drawing 2–2 with Crystal Palace.
Miller's squad number ranged from 6 to 8. In October 2010, he was named PFA Fans of the Month in League One, notably teammate Nicky Weaver.
Miller was fired from his employment on May 10, 2011. Miller retaliated against Irvine replacement Gary Megson for compeling him out of the team, and Miller believes Megson gave Miller a new deal after his release.
Miller signed a one-year deal with Huddersfield Town on July 7th. Miller was invited to join Gateshead's Conference National team before joining Huddersfield Town. In the 1–1 draw against Bury at the Galpharm Stadium on August 6, 2011, he made his Terriers debut as a replacement.

In a 2–2 draw against Scunthorpe United at Glanford Park on October 25, 2011, his first league goal was scored.
He left the club in June 2012 after not being offered a new deal by the manager, Simon Grayson, after being refused a new one.
Miller was unveiled on June 21, 2012 as the new manager of Swindon Town and Manager Paolo Di Canio. Miller joined the club on a free transfer for a one-year contract. Gary Roberts, a former Terriers teammate who signed a few days earlier at The County Ground, brings significant experience to the team. . Hartlepool United's former club Hartlepool United was one of the clubs interested in signing Miller, and he had been in touch with him since his release. He scored his first goal for Swindon on August 21, 2012 against Crawley Town at the County Ground.
Miller and Darren Ward will be largely in charge of the squad for the game against Preston on February 20th.
Miller joined Bury on August 19, 2013, with Kevin Blackwell saying, "Tommy brings a wealth of experience that is lacking at this level." He's a natural performer on the ball and is one of the key players I've been looking for to join the club. He had had offers at higher levels, but he was really impressed with Gigg Lane's new look. He'll be a tremendous asset to the team."
At the end of the 2013–14 season, Miller left Bury before his deal came to an end.
Miller left Bury at the end of the 2013–14 season to join Hartlepool United under Colin Cooper's guidance. In October 2014, he strained his calf but made one more appearance, against Exeter City, which confirmed Hartlepool's Football League status. This was also his 600th appearance in his career.
