Blair was involved in a T.S. production in 1990 while she was at Cranbrook Kingswood. Eliot's Murder in the Cathedral. She thought it was a failure, but her English teacher told her not to give up; this was the first time she felt she might be an actress. She began attending acting classes in New York in 1993. Blair subsequently signed with her after an agent discovered her in an acting class. In the mid-1990s, Blair first started auditioning. She had attended her first audition for a cereal company. She appeared in her first professional role in 1995 on the children's television sitcom Adventures of Pete & Pete, in which she portrayed Big Pete's love interest. She appeared in The Broccoli Theory, a "unromantic comedy" set in New York in 1996. In 1997, she made her first appearance in a mainstream feature film, the Kevin Kline comedy In & Out. She auditioned six times for the role and stayed on set for several weeks, but the bulk of her scenes were cut from the film's final version. She made her debut in a teen drama Strong Island Boys, based on true events involving a Long Island '80s street gang. Blair, according to actor Alec Baldwin, is "a cross between Debra Winger and Marlene Dietrich."

Later, she was chosen to lead the Amazon High fantasy film, co-starring Karl Urban. She was a present-day orphaned high school student who mistakenly traveled back in time to the Amazons' golden days. The film, which was also listed as a third show set in the Hercules and Xena mythological genre, has never been broadcast. Portions of the pilot were later converted and edited into the Xena: Warrior Princess episode "Lifeblood" in 2000. Blair was cast as Joey Potter on Dawson's Creek in the same year, but Katie Holmes replaced him later. Sarah Michelle Gellar was selected shortly afterward for the role of Buffy the Vampire Slayer, but she was refused. In the family drama No Laughing Matter as a pregnant teenager, Blair appeared opposite Suzanne Somers. She appeared in a number of independent and short films, including Debutante, opposite Josh Hartnett, which was shot in NYC and received numerous accolades after its premiere in September 1998.

Blair appeared in the teen drama Girl in 1998 and in Brown's Requiem, based on the crime novel of that name, he starred Dominique Swain. She appeared in "Charmed" (which was also the opening theme for the first season of Zoe, Duncan, Jack, and Jane in 1999); and in the music video for the British alternative rock band Placebo's single "Everybody Me" that was featured on the Cruel Intentions soundtrack.

In the crime drama Highway, she appeared with Jared Leto and Jake Gyllenhaal, which was then immediately available on DVD in March 2002. The independent film, which was shot in Seattle, is set in the mid-90s grunge music scene and follows a road trip to the 1994 Kurt Cobain vigil. In the comedy The Sweetest Thing, Blair Diaz's roommate appeared alongside Cameron Diaz and Christina Applegate. Although the film received mixed reviews, it still earned US$68.6 million worldwide. She was nominated for the Teen Choice Awards in the "Best Actress in a Comedy" category for her participation.

Blair appeared on Rolling Stone's cover and in a television commercial for The Sims Online video game in 2002. In addition, she appeared in the episode "The One with Christmas in Tulsa" on the hit sitcom Friends. In Guillermo del Toro's blockbuster fantasy film Hellboy, co-starring Ron Perlman, Blair played Liz Sherman, a wounded pyrokinetic superhero. Critics praised the film based on Mike Mignola's well-known comic book series; The New York Times reported, "Blair's heavy-lidded eyes appear to be at half mast from some lovely lewd fantasy." She has a sleepy carnality and a dry, tense timing, making her a perfect foil for Mr. Perlman's straightforward bravado." In its first weekend, Hellboy dominated the box office in the United States and Canada, grossing US$99.3 million around the world.

Blair appeared in John Waters' satirical sex comedy A Dirty Shame, as well as Tracey Ullman. A.V. The film had a limited theatrical run in North America, receiving general mixed feedback. Blair was cast in a recurring role on Netflix's science-fiction drama series Another Life on August 21, 2018. Before being cancelled, the program lasted for two seasons from 2019 to 2021.
