Kid Wikkid's founders were based in Peterborough, Ontario. After hearing of the band's age and unaware of their ages, 14-year-old Bach auditioned for the group and was recruited by guitarist and band leader Jason Delorme. Kid Wikkid returned to Toronto, and Bach's father eventually allowed Bach to move in with an aunt.

Skid Row formed in the mid-1980s with lead vocalist Matt Fallon leading the group, and the group began playing at various clubs around the New Jersey area. In 1987, Fallon joined Skid Row in search of a lead vocalist. Bach was seen playing at Mark Weiss' wedding by Jon Bon Jovi's parents, who then contacted him and advised that they contact their son's friend, Dave Sabo, who was looking for a lead vocalist for his band. After briefly being a member of Detroit's Madam X, Bach was wary of joining another US-based band. Bach went to New Jersey to audition and became the band's lead vocalist after hearing Skid Row's demo tapes.

Skid Row's first two albums, Skid Row (1989) and Slave to the Grind (1991), became the first two albums to be on the Billboard 200, the latter of which debuted at number one. Subhuman Race (1995) by the band's third album was also highly praised.

Bach has been chastised for a photograph taken in 1989 by the T-shirt saying "AIDS Kills Fags Dead," a play on the insecticide Raid's tagline, "Red Kills Bugs Dead." In a 2003 interview, he said about the scandal: "It was really stupid and wrong for me to wear it for one-half an hour in my life." What nobody remembers is in 2000, when I was in 'Jekyll & Hyde,' and at an auction for Broadway Cares, I donated $12,000 of my own money to fight AIDS.'

Bach was dropped from the band in 1996 after differences over musical details and infuriating Bolan. However, rumors that he had left the band due to his other band members' refusal to appear as an opening act for Kiss were circulated. Rachel Bolan, a band, also had a side project, Prunella Scales, who were playing at the same time as the planned Kiss performance. The split between Bach and the other band members culminated in his departure from Skid Row. Skid Row was one of the first acts on the 2000 Kiss Farewell Tour four years ago, without Bach.

Bach toured with Triumph straight after the band's release of their album Edge of Excess, with lead vocals until the band broke up later this year.

Bach founded The Last Hard Men, an alternative rock supergroup formed by Breeders lead guitarist Kelley Deal, which also included Frogs guitarist Jimmy Fleming and Smashing Pumpkins drummer Jimmy Chamberlin in 1996. The band produced a full-length self-titled album for Atlantic Records but later decided not to release it. It was the first time a fanfare and a very limited pressing of 1,000 CDs were sold in 1998 on Kelley Deal's label, Nice Records. For the 1996 film Scream's soundtrack album, the band also recorded a cover of Alice Cooper's "School's Out" for the band's soundtrack album.

Bach's debut solo album Bring 'Em Bach Alive! in 1999, his first release after he was banned from Skid Row. It was mainly a live album of Bach's Skid Row songs, but it also released five new studio albums, including the single "Superjerk, Superstar, Supertears."

Bach began performing in Broadway productions in 2000. In April 2000, he made his Broadway debut in Jekyll & Hyde, where he played the title role(s) in the rheube. In 2001, Riff Raff appeared in The Rocky Horror Show as Riff Raff. On November 28, 2001, Bach appeared at New York Steel, a benefit concert held in reaction to 9/11.

In early 2002, he became the host of VH1's Forever Wild. Bach was invited to appear in Jesus Christ Superstar's national touring production in October of that year, alongside JCS veteran Carl Anderson in the title role.

In June 2004, a DVD video of live shows named Forever Wild was released. In another performance of Jekyll & Hyde that same year, he reprised the title role(s).

Bach tried out for Velvet Revolver before the band found Scott Weiland, but it was turned down, according to Slash, "We sounded like Skid Roses." Bach appeared on WB television show Gilmore Girls as "Gil," the lead guitarist in Lane Kim's band, Hep Alien, from 2003 to 2007. Members of Bach Tight Five (which was started by Bach in 2004, but later ended in a short time) lived with Bach and his family on VH1's "I Married..." collection. Bach performed on An Absence of Empathy, a Frameshift record that was released in April 2005. He was directed to Henning by Dream Theater's James LaBrie.

Bach performed "My Michelle" on stage at the Hammerstein Ballroom in New York City on May 12 and 14, 2006. For the third time since May 15th, he performed "My Michelle" with Rose and the rest the following night. He also appeared at their Pre-Download Festival performance in the Apollo Hammersmith, London, singing My Michelle. After 13 years of not speaking, Rose introduced Bach, who said that the two had rekindled their friendship in the previous week. On June 9, 9 & 11, Rose joined Rose on stage at the 2006 Gods of Metal Festival in Dublin and Donington, respectively. He appeared on stage again at Inland Invasion festival in California on September 23, 2006, for a version of "My Michelle."

Bach appeared on the 2006 VH1 reality show Supergroup, in which he, Ted Nugent, Scott Ian, Jason Bonham, and Evan Seinfeld were all charged with forming a heavy metal supergroup, with Bach as the lead singer. The seven-episode series was shot in Las Vegas during a twelve-day period when the musicians all lived together in a mansion. The resultant band, originally called "Fist," was eventually named Damnocracy; both bands were Bach's inventions. There was no evidence of misdemeanors in Damnocracy.

In 2007, Bach and EMI announced a joint venture to jointly produce a brand owned by him, including his album Angel Down, which was released on November 20, 2007. Bach also appeared backing vocals for the track "Sorry" on Guns N' Roses' long-delayed Chinese Democracy, which was first announced in November 2008. He spent the summer of 2008 with Poison and Dokken.

He appeared on the 2007 MTV reality show Celebrity Rap Superstar, finishing third place.

Bach was the champion of the second season of CMT reality show Gone Country, which premiered in 2008.

Bach appeared on "Chinese Democracy Tour" in 2009–2010, and he performed "My Michelle" with Axl Rose in Quebec City on February 1, 2010. He appeared on NBC's Jimmy Fallon Show on January 5, 2011 as a power ballad written for the University of Oregon Ducks and a follow-up video of "We Are The Ducks," a power ballad written for the University of Oregon Ducks. Bach was interviewed by British metalcore band Asking Alexandria in the Revolver's March/April issue. The band is a fan of Skid Row and they've covered two of their songs in the preceding year. Bach performed "Youth Gone Wild" with the band live at Revolver Golden Gods Awards and Rock on the Range in the same year. Bach was also shot in their music video "Closure."

In the SpongeBob SquarePants episode "SpongeBob and the Clash of Triton," Bach played Prince Triton, King Neptune's rebellious son. Bach revealed Kicking & Screaming, the 2011 title of his forthcoming solo album. The first single's track list, cover art, and title were revealed on July 8, 2011. For North America and Europe on Frontiers Records, it was released on September 27, 2011 and September 23, 2011. Nick Sterling was fired by Bach on August 13, 2012, after refusing to sign an agreement to appear on an undisclosed TV show.

Big Noize was a 2007 all-Star band made up of founder and drummer Vinny Appice (Black Sabbath, Dio, Heaven & Hell), as well as other "monsters" such as guitarist Carlos Cavazo (Quiet Riot), bassist Phil Soussan (Ozzy Osbourne) and vocalist Joe Lynn Turner (Rainbow). Turner was back on display at the Kavarna Rock festival on July 14, 2012, as well as a tour to South America, beginning in Chile on July 9, 2013.

Bach revealed on Twitter on April 30, 2013 that a new studio album was in the works. Bob Marlette would be back as producer, according to him.
