Bettany began a three-year course at the Drama Centre London in Chalk Farm in 1990. At the Aldwych Theatre, he made his stage debut in Stephen Daldry's highly acclaimed West End revival of An Inspector Calls. He appeared as Eric Birling in Stephen Daldry's acclaimed West End revival. He appeared in Richard III, Romeo, and Juliet's productions, as well as Julius Caesar (for which he received a Charleson Award nomination). When Bettany was 21 years old, he appeared in a BBC film starring Bill Sikes.

In the feature film Transcendence, Bettany appeared alongside Johnny Depp and Rebecca Hall. He was paired with Depp in Mortdecai, a 2015 motion picture starring Gwyneth Paltrow and Ewan McGregor. In the Discovery Channel television show Manhunt: Unabomber, Bettany portrayed Ted Kaczynski. After Williams was booted from the final film because he was unable to return to set during the film's reshoots, Bettany replaced him with Michael K. Williams in Solo: A Star Wars Story. Bettany was cast in his role, with the image being reworked from a motion-capture alien to a human. Bettany was close to completing Matt Smith on Netflix's The Crown in January 2021. Bettany was eventually unable to sign on due to scheduling conflicts days later, on January 25th.

Bettany appeared alongside Elizabeth Olsen in the Marvel television miniseries WandaVision, presenting a new interpretation of his Marvel Cinematic Universe character, Vision, with Olsen portraying the titular Wanda. Critics lauded it when it premiered on Disney+ in January 2021. Matt Purslow of IGN said Bettany "effortlessly contributes to the comedic skits" and that he and Olsen "provide a slew of life, wisdom, and emotion." Bettany was nominated for the Primetime Emmy Award for Outstanding Lead Actor in a Limited or Anthology Series or Film, as well as the Golden Globe Award for Best Actor – Miniseries or Television Film.

Bettany appeared in A Very British Scandal, the 2021 miniseries. In a performance of Anton McCarten's The Collaboration by Andy Warhol at the Young Vic in London from February to April 2022, he played artist Andy Warhol. He is expected to reprise his role in a film adaptation of the play. In a film about the Facebook–Cambridge Analytica data scandal, as well as co-write and star in the dramedy film Harvest Moon, Bettany is attached to play businessman Alexander Nix.
