Djokovic was one of "a handful" of players and staff to be granted a medical exemption by Tennis Australia and the Department of Health in Victoria. It was later announced that Djokovic tested positive for COVID-19 on December 16, 2021, which was used as the basis for his exemption.

On November 18, 2021, Djokovic was given a visa to enter Australia. He travelled to Melbourne on January 5th, but the Australian Border Force arrested him on the grounds of his declaration that he did not comply with the entry requirements for an unvaccinated traveller on the grounds of which he was refused an exemption. Djokovic admitted that a member of his support staff ticked a box on his application form stating that he had not traveled abroad two weeks before he left for Australia; however, he had not been to Spain at the time. His visa was cancelled and he was detained in a detention facility for several days in anticipation of a court hearing.

Djokovic requested a judicial review, but three Federal Court of Australia judges have unanimously refused his application, effectively ending his participation in the 2022 Australian Open. Djokovic said he was "terribly distraught" with the decision but praised the decision and flew from Australia to Dubai that night. Djokovic said in February that he would not have a COVID-19 vaccine despite his removal from Australia and said he would not worry about his work by sticking to his values of free choice and not having the COVID-19 vaccine. He faced Rafael Nadal in the quarterfinals after defeating Yoshihito Nishioka, Alex Molan, Alja Bedene, and Diego Schwartzman in straight sets.
