Before Grahame appeared on Big Brother, she appeared as an extra in the BBC One soap opera EastEnders and played a footballer's wife in Sky One's Dream Team. She appeared on the ITV dating show Blind Date in 2003. Grahame, who had received an NVQ in beauty therapy, competed in the 2004 Miss Hertfordshire pageant, placing fourth.

Grahame became a contestant on Channel 4's seventh series of Big Brother in which she was known for her physical appearances such as temper tantrums, Diary Room theatrics, and being intimately linked to Pete Bennett, the series's winner. Grahame suffered with anorexia nervosa for the majority of her childhood and had attempted suicide by taking an overdose of ketamol during her tenure as a Big Brother contributor. Experts sluggishly condemned the programme's decision to include a contestant with prior mental disorder diagnoses; experts argue she should not have been allowed to participate. Grahame, on the other hand, confessed to intentionally concealing her diagnoses and psychophysiology from the Big Brother team, but only revealed the truth when she was still in the House and struggling.

Grahame was nominated for eviction four times during her first stay in the Big Brother house and was eventually evicted on Day 58 after winning 37.2% of the public vote. By 63% of the voting public, she and three other ex-housemates were accepted into the "House Next Door" less than four weeks after she had been barred from the house of Commons. Although she had already been banned, the remaining contestants chose Grahame to return to the main house. Grahame was deemed an eligible housemate and was given the opportunity to win the show's £100,000 draw once more, angering 2,700 viewers to report the show's premium-rate telephone watchdog ICSTIS.

Grahame finished fifth in fifth place with 6.5 percent of the final vote. The crowd's reaction was less encouraging than it had been at her previous eviction when she led to unanimous chanting and jubilation as she began. Samantha McCall attempted to interview Grahame but she was reluctant to talk, saying she was afraid and shocked by the crowd's booing. Grahame's live interview was cut short, and she was led off the stage.

Grahame spoke at the "Celebrity Spotting" segment on one episode of The Friday Night Project, which was usually presented by Debra Stephenson. Grahame has also co-produced several episodes of Celebrity Soup with Iain Lee on satellite channel E! Entertainment Television. Grahame would appear in a reality television series similar to Simple Life in which she was evicted. The program followed her attempts to hold a regular job while being inspired by her failed stint as a personal assistant in a Big Brother role. Princess Nikki debuted on E4 on September 6th and lasted for six weeks.

Grahame made personal appearances, including appearances at various television shows, such as Celebrity Scissorhands and 8 out of ten Cats, as well as appearances at student nights and clubs. Grahame appeared on a limited "Goodies and Baddies" version of the game show The Weakest Link on March 24, 2007. She was the best link of the team during round three (answered the most questions correctly and received the most money), but she was disqualified in round five after becoming the team's poorest link. In May 2007, Grahame competed in the Soccer Six charity tournament at Wrexham's Racecourse Ground.

Grahame returned to Big Brother as a reporter for Big Brother's Little Brother in June 2007. Grahame and several other former housemates were sent by "The B-Team" to question people about Big Brother 8. In addition, she wrote a Big Brother column in the magazine OK!. Grahame competed in Ultimate Big Brother on August 24, 2010, becoming the only homemate to represent Big Brother 7. Grahame was nominated for a double eviction in the show's second week, alongside five of her housemates. After polling less than half of the other candidates, Grahame escaped the general election. Grahame was the show's runner-up on September tenaciously after losing to Big Brother 2 winner Brian Dowling in the final election. Grahame placed three places higher than her placing in Big Brother 7 at the top of the charts.

During Grahame's time in Ultimate Big Brother, she seemed to be more comfortable discussing her anorexia; in particular, she talked to Vanessa Feltz, who has also had weight issues.

Grahame appeared in the sixteenth season of Big Brother as a "time warp housemate." She collaborated with JYYUK London in September 2015 to produce a line of fashionable clothing for petite people. Grahame wrote an article for Reveal magazine: 'In an interview with a magazine, Grahame said: 'It's been here for a long time.'

Grahame was named one of four 'international wildcards' representing Big Brother UK on Sunday, and he could have been elected into the Big Brother Canada house by an online vote on February 23, 2016. Following Grahame's first eviction on March 3, 2016, it was revealed that the two women, as well as Tim Dormer, the winner of the tenth series of Big Brother Australia, had been elected into the house. Both of them were saved from eviction for the week. Grahame was chosen in the top two Canadian candidates for the week's Head of Household (HoH), but she lost a competition to fellow housegues Nick & Phil Paquette on April 14th. HoH Cassandra nominated Grahame for eviction as a "pawn" to evict a larger opponent on April 24th. She survived this eviction attempt on April 28th, when Jared Kesler was evicted in a 3:1 eviction election. Grahame was also selected for removal by HoHs Nick and Phil alongside Dormer in a double-eviction twist. Dormer was vetoed off the block and was replaced by Joel Lefevre. Grahame was voted tenth in a 3:0 eviction vote and received a standing ovation from the studio audience. On Day 63, Grahame's 34th birthday, she was expelled. Grahame was the fifth member of the nine-person jury that selected the season's winner on Finale Night as a result of her removal. She and six other jurors voted for Nick and Phil to be the series's champions.

Grahame appeared on Channel 5's "In Therapy," which aired on August 2017 in August. Grahame appears in intensive, on-camera therapy sessions with psychotherapist Mandy Saligari, during which Grahame talked about her depression and anorexia.

Grahame appeared on the Big Brother's Bit on the Side panel a number of times. Grahame was a special guest at the Big Brother Canada awards in 2017. For the final time, Grahame entered the Big Brother house in 2018. Grahame said that it was the last series in the series, and he was optimistic about it being the last series.
