In the 1981 film version of Bob Randall's novel The Fan, starring Lauren Bacall, he appeared in the role of Douglas Breen, a stalker. Biehn played Sgt. In 1984, he first appeared as a kid. In the movie The Terminator, opposite Arnold Schwarzenegger and Linda Hamilton, Kyle Reese, a soldier sent back in time by John Connor to save his mother, Sarah Connor. He appeared in two other James Cameron films, including Aliens (as Corporal Hicks) and The Abyss, and played Reese in a small part in Terminator 2: Judgment Day, although Reese was briefly reprised in a scene cut from the final film but was restored for the Director's Cut version. He was supposed to play the T-1000's villain, but Robert Patrick took the lead. In an interview, Biehn confirmed that he would not be reprising the role of Terminator: Dark Fate or any other upcoming sequels. Biehn confessed that he was initially worried about being in a Schwarzenegger film because he wanted to appear alongside Al Pacino and Robert De Niro.
In an early draft of Alien 3 written by William Gibson, Biehn's character Hicks (who had survived the events of Aliens) was supposed to be the protagonist, replacing Ellen Ripley (Sigourney Weaver). The final script was written by Walter Hill and David Giler, who had Hicks killed off in the first scene. Biehn, who learned of his character's death, requested and received almost as much money for the use of his likeness in one scene as he had been compensated for his work in Aliens. Biehn recalled the role of Hicks in the video game Aliens: Colonial Marines and later in 2019 for an audio drama version of Gibson's un-filmed script for Alien 3. With the showdown scene starring Val Kilmer as Doc Holiday, Biehn played Johnny Ringo in Tombstone.

He appeared in three TV series, including the CBS drama The Magnificent Seven (1998–2000), the Tribune Entertainment syndicated TV series Adventure Inc. (2002–2003), and the NBC TV series Hawaii (2004). Because of poor ratings, both three shows were subsequently cancelled. Biehn was supposed to portray Colonel Miles Quaritch, James Cameron's top antagonist in James Cameron's science fiction epic film Avatar (2009), but Cameron worried that his presence in the film, as well as that of Sigourney Weaver, would remind people too much of Aliens.
The Blood Bond, directed by Biehn in 2010, was released in Germany. In 2011, he wrote, directed, and starred in The Victim. In the second season of the Star Wars television series The Mandalorian, Biehn was cast as the villain Lang. Ian was portrayed in the eleventh season of the AMC horror film The Walking Dead in 2022.
