In the 1980s, Dillon appeared on television and films. In 1983, Dillon appeared in Arnold Norberry's television film No Big Deal. Ed Rooney was in the film Heaven Help Us, Michael Dinner's first big role. In the 1986 film Platoon, he attracted a lot of attention for his role as the psychopathic soldier Bunny. In 1988, he played Brian Flagg in a remake of The Blob, a 1958 American science fiction film. It received mixed feedback, but it was praised for its unique functionality and has since gained cult following. War Party (1988), No Escape (1994), and The Doors (1991), in which he played drummer John Densmore, were two other notable film roles from that period.

The bulk of Dillon's work in the 2000s was spent on television, including the CBS series That's Life as Paul DeLucca and the Fox series 24 as Lonnie McRae. He appeared in Poseidon, a remake of The Poseidon Adventure, in 2006. In April 2010, he appeared on Donald J. Trump's Fabulous World of Golf on The Golf Channel. He appeared in the family comedie film Hotel for Dogs (2009), which earned $117 million at the global box office.

In 2007, 2008, and 2009, Dillon co-starred on the HBO comedy drama television series Entourage playing Johnny "Drama" Chase, for which he received Emmy nominations. In 2008, he received a Golden Globe Award for his appearance on the program. After eight seasons, the series came to an end on September 11, 2011. In the series' film version, Entourage (2015), Dillon reprised his role as Johnny Chase. The project was officially announced in 2013, and filming began around Los Angeles in February 2014. It received mixed feedback from critics and took in $49 million globally.

In 2011, Dillon appeared in the CBS comedy sitcom How to Be a Gentleman portraying Iraq war veteran Bert Lansing. Jimmy O'Shea was cast in the drama film Dirt and portrayed him in two episodes of police procedural drama series Blue Bloods in 2018.
