In 1996, Monaco, who had been dreaming of a career in modeling, sent her photograph to Playboy. As a result, she was named Playmate of the Month for April 1997. Monaco was also included in several Playboy Special Edition publications during her time with Playboy in the 1990s.

From 1997 to 1998, Monaco's first television appearance was in the drama Baywatch series. Monaco was also Carmen Electra's body double at times, as Electra was unable to swim. Susan on the program was also Susan on the show. Monaco appeared in BASEketball, Idle Hands, and Mumford in the late 1990s.

Monaco played two roles in the ABC soap opera Port Charles: Olivia "Livvie" Locke Morley (1999-2003) and Tess Ramsey (2002–03). Samantha McCall joined the ABC soap opera General Hospital as Samantha McCall in October 2003 as a volunteer.

Monaco was nominated for the Outstanding Support Actress in a Drama Series in 2003 for her role on Port Charles. Despite being initially subjected to stern reviews from the judges, she maintained a positive mood throughout the series. Among the activities she attended before her victory was a wardrobe malfunction in which the skimpy strap on her dress fell off during a Latin dance number.

In 2005, Monaco appeared in a Maxim cover story, and the magazine also ranked her at #13 on the annual Hot 100 List in 2006.

Monaco boosted her dancing resume with Peepshow, a burlesque performance directed by Tony Award-winning producer Jerry Mitchell, who appears at the Planet Hollywood Resort and Casino in Las Vegas. Bo Peep, a precocious character who learns her sexuality throughout the series, starred Monaco. Melanie Brown, a former Monaco contestant, appeared alongside musician and fellow Dancing with the Stars until June 2009, when Monaco's deal came to an end and she was replaced by Holly Madison. Monaco was named Maxim's top sexiest cover model of the decade last year in the same year.

Monaco would be competing in the 15th season of Dancing with the Stars for the chance of winning a second mirrorball championship on July 27, 2012. This time she was partnered with Valentin Chmerkovskiy. Monaco finished third place on November 27, 2012, after qualifying for the second time.

Billy Miller, a director of GM, died on September 17 at the age of 43 after a battle with'manic depression.' According to soap opera journalist Michael Fairman, the soap opera singer and three-time Emmy winner, who also appeared on The Young and the Restless, died on Friday in Austin, Texas. As per TMZ, the reason for death has yet to be confirmed, but reps for the actor said he was suffering from'manic depression' at the time of his death.
