Small lives in West London. Rio Ferdinand Foundation and Greenhouse Sports are two charities that she supports. She had a long-term association with rugby player and mentor Shaun Edwards, with whom she has a son, Labour Party councillor James Small-Edwards. In 2012, she married lawyer David Neita.

Small was born and raised on a council estate in Ladbroke Grove, West London, by the time of George Small. She formed Hot House, her first band, while a teen, before she was a member. She appeared on Black Box's rerecorded version of "Ride on Time" in the studio. She had a chance meeting with Manchester DJ Mike Pickering, a founding member of Quando Quango, which resulted in the company's successful M People album sales of over ten million albums worldwide. They had a lot of success with songs including "Moving On Up," "One Night In Heaven," and "Search for the Hero."

Small appeared in "Perfect Day" the official 1997 Children in Need charity launch, alongside a large number of artists, including Tom Jones and Lou Reed. It was the UK's most popular single for three weeks after selling over a million copies.

Small and M People took some time off to pursue solo ventures after The Best of M People was announced in 1998.

Small's debut solo album, Proud, and a single of the same name in 2000, which was closely linked to the British Olympic team of the time. Small's most popular song, which was used for the first season and the final episode of Queer as Folk's final season, is the single, which has had international success. When Oprah Winfrey was looking for a song to sum up her effort over her 20-year career, she contacted Small, and "Proud" became the show's theme tune; in 2005, Small made her American television debut on the program. "Proud" became the theme tune for the American reality weight loss show The Biggest Loser later this year.

As the next single, "You Need Love Like I Do" was released in November 2000, it was followed by a duet single with Tom Jones in November 2000.

Small took time off to look after her newborn son and record her second solo album. She appeared on several television shows, including Parkinson and Songs of Praise. At the Millennium Stadium in Cardiff, she performed three songs at the Tsunami Relief Concert.

Ultimate Collection, M People's second best hits collection, was launched in 2005. Both of M People's hits and two of Small's solo debuts were included on the album. "Proud" was re-released from this album and ranked third on the UK Singles Charts. M People reunited and played on an arena tour in 2005, and they have played a handful of times a year since.

On July 17, Small launched "Radio On" a new single in the United Kingdom. Close to a Miracle, her second solo album, was released on July 24th, and a second single of the same name was released digitally on September 25, 2006. "Changes" was also included in the album, which had not been announced. The album was a commercial disappointment—it topped the charts for one week at number 57, and both supporting singles failed to reach the top 100—but a short UK solo tour later this fall was well-received.

Small performed "Luna" with Icelandic tenor Garr Cortes in 2007. It was released as a single from his album Cortes and ranked second in the Icelandic online music charts. On the occasion of 200 years since the slave trade was abolished, Small performed at a concert in London on September 10, 2007. Small appeared with Stereophonics and Feeder at the Millennium Stadium in Cardiff on May 26, 2008, as part of Wales' 2008 Grand Slam success. In addition, Small performed three nights at Ronnie Scott's in October as part of her 2008 tour, and headlined Manchester Pride.

She appeared at the Visa London 2012 Party on August 24th, 2008, to mark the handover as the host city of the Olympic Games from Beijing, China, to London. Small performed "Proud," the unofficial anthem of Team GB at the Athens Games in 2004, and it was the official anthem of the London 2012 Games. On the 2008 BBC Sports Personality of the Year program, she performed "Proud" again as a soundtrack to a montage of British Olympian accomplishments in Beijing.

Small was one of the sixteen celebrities in BBC's sixth series of Strict Come Dancing, where she was partnered with newcomer Brian Fortuna. In weeks 4, 5, 7, and 8, the couple were ranked ninth overall in the series and were in the bottom two on four occasions. They were then disqualified by the judges after losing in the dance-off against Rachel Stevens. Small also appeared in Dancing on Wheels, a BBC Three film.

Small toured the UK from late 2008 to 2009. Small's solo albums, M People songs, and a few jazz covers were among the setlists.

Small was revealed in July 2010 that Chaka Khan would be sharing Chaka Khan's role with Anastacia and Lulu for the second Here Come the Girls tour, which took place across the United Kingdom between November and December 2010. In recognition of her contributions to music, she was given a BASCA Gold Badge Award on October 20th.

Small appeared as half-time entertainment at Twickenham Stadium in London for the annual St. George's Day Premiership Rugby fixture, hosted by Wasps in April 2011. Small performed "Proud" and "Look for The Hero," as well as a backing dance troupe of 546 UK students, earning the Guinness World Record for a Popstar."

Small performed "Proud" at the 2012 Pride of Britain Awards in London on October 29, 2012, as a salute to the athletes of the London 2012 Olympic Games.

Small, along with Neil Morrissey, Debbie McGee, Ed Byrne, Raphael Rowe, and JJ Chalmers, participated in the Camino de Santiago pilgrimage for the BBC in 2017. "I was a believer before I set off, but this trip made me realize my faith and beliefs were much greater than I had expected."

Small's in 2018 celebrated 25 years of performing. She toured around the United Kingdom, celebrating her and M People's music.

Small had intended to debut Heather Small: Orchestral Greatest Hits in May 2018, but the venture was cancelled on May 3rd.

Small toured from 10 April to April 26, 2019.

Small appeared on the third series of The Masked Singer in January 2022 as "Chandelier." She was the first person to be unveiled.
