Stanhope began his comedy career in 1990 at the age of 23, first appearing at an open mic night in a Las Vegas bar. He began his Las Vegas act before moving to Phoenix, Arizona, where he found a job as a house MC at a comedy club.

He settled in West Hollywood in 1995, partly because of the time's comedy shows's downturn. Before becoming based in Playa del Rey and playing in bars outside of the city center, he was one of his regular spots, the Hollywood Improv or The Comedy Store. He landed a development contract and was "flirted" with the prospect of releasing a sitcom by co-writing a pilot episode with a writer of The Mary Tyler Moore Show. Later, he described the script as a "piece of shit" and wished that it was never picked up. Stanhope defeated Dane Cook in a three-week competition in 1995.

Stanhope's remarks regarding the incident were criticized by the public in the aftermath of the September 11 attacks in New York City. Demonstrators from within his bands and on stage were involved in this.

In July 2002, Fox broadcasted back-to-back episodes of Stanhope's clandestine camera television show Invasion of the Hidden Cameras. Stanhope wrote and produced several pranks for it before the network decided to cancel its broadcasting. Stanhope accused Spy TV of stealing half of his ideas and receiving good ratings as a result. On Spy TV, he appeared as one of the pranksters.

Stanhope received the Strathmore Press Award in August 2002 for his week-long participation at the Edinburgh Festival Fringe, according to 40 press analysts. In the British celebrity magazine OK!, the winner was £1,000 and a feature. Stanhope has been working with his longtime manager Brian Hennigan after the initial vacancy.

Stanhope hosted the fifth and sixth seasons of The Man Show with Joe Rogan in 2003 and 2004. In order to be more financially stable, he committed to the initiative. Stanhope lasted five rounds in a boxing match with Tonya Harding, in which the 5'7" Stanhope lost by choice and his mother reviewing pornographic films were among the show's segments. Stanhope began The Unbookables, a touring comics collection that featured artists including Andy Andrist and Brendon Walsh, among other things. Morbid Obscenity, Unbookables' first album, was released in 2006.

Stanhope was booked to appear at numerous shows at the Cat Laughs Comedy Festival in Kilkenny, Ireland, in June 2006. However, the audience expressed a hostile response shortly after a joke about Irish men committing pedophilia as a result of their ugliness of Irish women. Stanhope's remaining appearances at the festival were cancelled due to the incident. He returned to Edinburgh Festival Fringe in August and committed to a full three-week stay. One of his routines sparked a violent confrontation with an outraged audience member and the police that was called in, causing Stanhope to leave the venue. He has received five-star reviews from the media. He took what seemed to be an ecstasy tablet that had been handed to him by a spectator on his first night. Stanhope introduced A Day with Doug, the average amount that comedians lose when booking at one of the festival's four major venues.

Stanhope declared that he intends to run for president as the United States president in the 2008 race on the Libertarian Party ticket. "It seemed like a funny thing to do [...] But people were either amused or horrified at the prospect of me representing their party," he said. His concern was for "individual rights," self-government, and making America fun again.

By London's The Guardian newspaper, his live show was ranked in the top 5 of the 20 Best Live Shows of 2009.

From Across The Street, Stanhope's seventh album, was released on November 24, 2009.

Stanhope produced a series of vignettes for Charlie Brooker's British news show Newswipe in 2010. He did the same thing in 2013 as "the voice of America" during Charlie Brooker's Weekly Wipe as "the voice of America." Stanhope announced their new comedy group, Roadrunner Comedy, on November 17, 2010. The album was released on May 3, 2011. Burning The Bridge To Nowhere is a film that was released in Oslo. This was part of an international tour that included a 25-night stay at the Leicester Square Theatre in London. Stanhope had been named twice by this time in Time Out New York's Best Comedy Performance award.

Stanhope organized a performance in Iceland's only maximum security prison, Litla-Hraun, in 2011, in collaboration with Reykjavk's mayor and comedian Jón Gnarr. On September 25, the exhibition took place. Stanhope appeared on Louie as Eddie Mack, a British comedian. When they first started performing in "Eddie," the season 2 episode "Eddie" premiered on August 11, 2011. "I tried to beg out of it," he explained. I hate acting, as I said. "He had to talk to me into trying" and auditioned online via Skype.

Stanhope's comedy special Before Turning the Gun on Himself was released in Salt Lake City in 2012. He took the show's name from a word that he found on internet news articles. In another interview with Mike Ragogna in 2012, he impersonated John Lydon in a pre-taped radio interview, although the studio mistakenly called his number mistaking it for Lydon's.

Stanhope unveiled The Doug Stanhope Podcast on February 16, 2013, straight from his Bisbee, Arizona, home. The All Things Comedian network hosted it in 2014.

Rebecca Vitsmun and her family, whose house was destroyed by the 2013 tornado in Moore, Oklahoma, received almost $26,000 in May 2013. He did so in favor of her declared atheism, saying he admired her courage in a predominantly Christian nation.

Stanhope revealed on The Howard Stern Show in 2014 that he was working with Johnny Depp on a show. Depp befriended him in Europe and had an unknown project in mind for him, according to Stanhope. He appeared in the 2014 Chris Rock film Top Five, as a police officer.

On November 21, 2015, Stanhope filmed his comedy special No Place Like Home at the Bisbee Royale in Bisbee. Johnny Depp directed the film.
